耐阿奇霉素的社区获得性支原体肺炎的临床诊治

摘　　要：目的通过分组比对,总结耐阿奇霉素的社区获得性支原体肺炎的诊断与治疗经验。方法回顾性分析某院近3年确诊为支原体肺炎的21例临床资料,将所有病例随机分为莫西沙星联合头孢哌酮舒巴坦钠组10例和左氧氟沙星联合头孢哌酮舒巴坦钠组11例,分别采用静脉滴注疗法7～10d后,改为口服莫西沙星或左氧氟沙星序贯治疗2周,观察两组的临床疗效和不良反应。结果莫西沙星联合用药组和左氧氟沙星联合用药组治愈率分别为80%和72.7%,所有病例用药结束后治疗有效,2组治愈率和有效率比较差异无统计学意义(P﹥0.05),临床症状和体征好转时间2组差异也无统计学意义(P﹥0.05),不良反应发生率均低,未影响药物治疗。结论莫西沙星组和左氧氟沙星组序贯疗法治疗支原体肺炎疗效确切,不良反应发生少,患者依从性高,是治疗支原体肺炎的理想一线药物。OBJECTIVE To compare the curative effects by grouping treatment, summarize the diagnosis and treatment experi- ence of the community acquired mycoplasma pneumonia resistance to azithromycin. METHODS We analyzed the clinical mate- rial of 21 patients diagnosed with mycoplasma pneumonia for the past three years, 21 patients were divided into two groups stochastically： Moxifloxacin union cefoperazone sodium and sulbactam sodium 10 cases, and levofloxacin union cefoperazone sodium and sulbactam sodium 11 cases, use intravenous drip separately for 7 to 10 days, than take mycoplasma or levofloxacin orally sequential therapy 2 weeks, to observe clinical curative effects and adverse reactions in 2 groups. RESULTS The moxi- floxacin union medication group and the levofloxacin union medication group cure rate respectively is 80% and 72.7%, there was non-significance difference in cure rate and effective percentage between 2 groups （P〉 0.05）, also do not have the signifi- cance difference in the clinical symptom and the sign got better time between 2 groups, low adverse reactions rate. The medicine treatment has not been affected. CONCLUSION Moxifloxacin or Levofloxacin union medicine and sequential treatment have better effect for treating the acquired mycoplasma pneumonia. The adverse reactions rate was low, and the patient compliance was high and therefore moxitloxacin and levofloxaein are the ideal front-line medicines in treatment of the community acquired myeoplasma pneumonia.
