高压氧联合硝酸甘油对妊娠期高血压疾病疗效观察

摘　　要：目的通过高压氧联合硝酸甘油治疗妊娠期高血压疾病与单纯硝酸甘油治疗妊娠期高血压疾病的脐动脉血流收缩期最大血流速度／舒张期末血流速度比值（S／D比值）及阻力指数（RI）对比，探讨高压氧联合硝酸甘油在妊娠期高血压疾病治疗中的作用。方法选择2009年5月至2011年3月妊娠期高血压疾病待产妇60例，入院时完全随机分组：单纯用硝酸甘油治疗组（对照组）和高压氧联合硝酸甘油治疗组（观察组）各30例。对照组患者给予硫酸镁，同时静脉滴注硝酸甘油。观察组在对照组治疗基础上，加用高压氧进行联合治疗。治疗前及治疗后采用彩色多普勒测脐动脉S／D比值及RI。各组患者于治疗前、后测量平均动脉压（MAP）。结果观察组胎儿脐动脉S／D比值及R1分别为2．39±0．12及0．49±0．21，对照组分别为2．50±0．11及0．58±0．25；2组比较，差异有统计学意义（P〈0．05）。观察组MAP为（96±5）mmHg，对照组MAP为（102±5）mmHg；2组比较，差异有统计学意义（尸〈0．05）。结论硝酸甘油联合高压氧治疗妊娠期高血压疾病，降压疗效确切，能改变血流变、抑制血小板聚集、降低胎儿一胎盘循环阻力，对妊娠期高血压疾病引起的胎儿宫内发育迟缓可能有治疗作用。Objective To explore the effectiveness of hyperbaric oxygen combined with nitroglycerin in the treatment of intrauterine growth retardation. Methods Sixty cases were randomly divided into two groups： observation group and control group. The observation group was treated with hyperbaric oxygen and nitroglycerin, and the control group was given nitroglycerin only. Results The values of S/D and resistant index （RI） of umbilical arteries in observation group were 2. 39 ± 0. 12, 0. 49 ± 0. 21, and those in control group were 2.50 ±0. 11, 0. 58 ± 0. 25. There was a significant difference between two groups （ P 〈 0. 05 ）. The values of mean artery pressure （MAP） in observation group was （96±5 ）mm Hg, and that in controlgroup was （102 ± 5 ）mm Hg. The values of MAP in treatment group was more significantly reduced than that in control group after treatment. Conclusion Hyperbaric oxygen combined with nitroglycerin can Obviously reduce blood pressure and significant clinical effect can be obtained by means of hyperbaric oxygen combined with nitroglycerin in the treatment of intrauterine growth retardation induced by pregnancy induced hypertension.
