银杏叶片联合西咪替丁治疗小儿过敏性紫癜的临床研究

摘　　要：目的探讨银杏叶片联合西咪替丁治疗小儿过敏性紫癜及预防紫癜性肾炎的疗效。方法选择确诊为过敏性紫癜的患儿79例，就诊时均无肾脏损害。完全随机分为观察组（39例）与对照组（40例）。2组均给予常规对症治疗等，观察组另加用①西咪替丁8-10mg／（kg·d）加入5％葡萄糖注射液100-200ml中静脉滴注，1次／d，疗程10-14d；②口服银杏叶片：年龄≤5岁者1片／次，2次／d，年龄〉5岁者1片／次，3次／d，连续服用3个月。对比2组总有效率、临床症状消失时间及尿液中免疫球蛋白G、微量白蛋白、B2微球蛋白的变化。结果观察组和对照组总有效率分别为92．3％（36／39）和67．5％（27／40），组间差异有统计学意义（P〈0．01）。观察组治疗后尿液免疫球蛋白G、微量白蛋白、B2微球蛋白结果均低于对照组治疗后，组问差异有统计学意义[免疫球蛋白G：（6．9±2．7）mg／L比（10．2±7．6）mg／L，微量白蛋白：（17±7）mg／L比（23±13）mg／L，p2微球蛋白：（0．59±0．10）mg／L比（0．68±0．17）mg／L，均P〈0．05]。结论常规治疗的基础上早期使用银杏叶片联合西咪替丁治疗小儿过敏性紫癜及预防紫癜性肾炎有一定疗效。Objective To observe the clinical efficency of folium gingo combined with cimetidine in treating anaphylactoid purpnra and preventing anaphylactoid purpnra nephritis. Methods Seventy-nine children with anaphylactoid purpura were divided randomly into observation group and control group. Children in control group were treated with conventional and symptomatic treatment. Children in observation group were treated with oral folium ginkgo of one pill twice daily or thrice daily and cimetidine intervenous infusion of 8-10 mg/（kg-d） on the basis of routine for three months. The total effective rate, the extinction time of clinical symptom and the changes of immunoglobulin G （ IgG）, microalbumin and 132 microglobulin （β2-MG） in urine were observed in both groups. Results The total effective rate was 92. 3% （36/39） in observation group, which was significantly higher than the 67.5 % （27/54） in control group （P 〈0. 01 ）. The IgG and β2-MG level in urine after treatment in observation group were significantly lower than those in control group. Conclusion Folium gingo combined with cimetidine in the treatment of anaphylactoid purpura are effective and also can prevent the anaphylactoid purpura nephritis.
