糖尿病肾病与非糖尿病肾病患者血液透析前后中医证候差异观察

摘　　要：目的:观察糖尿病肾病与非糖尿病肾病患者在血液透析前后各时点中医证候分布差异。方法:选择开始进入血液透析的患者81例,根据中医辨证分型观察糖尿病肾病组和非糖尿病肾病组中的分布差异,并观察透析后各时点的证候变化规律。结果:透析前糖尿病肾病正虚证以气阴亏虚为主,实性证候主要为瘀血、水气、湿热证多见。血液透析后,阴虚证更为突出,湿热证呈先增后减趋势,水气证明显减少。证候积分逐渐减少,以实性积分更为明显。糖尿病肾病组与非糖尿病肾病组相比证候分布均存在明显差异。结论:糖尿病肾病有自己的证候分布规律,正虚证中阴虚表现突出,实证中湿热证多于非糖尿病肾病组患者,透析可明显改善患者症状。Objective：To observe diabetic nephropathy and non- diabetic nephropathy in hemodialysis patients with distribution difference of TCM syndrome in each point. Method：We selected 84 cases who accepted hemodialysis treat- ment. According to TCM syndrome,we observed differences in the distribution of TCM syndrome on diabetic nephropathy group and non - diabetic nephropathy group at each point, and syndrome change rule after dialysis at each point. Results ： Before dialysis,the syndrome of deficiency of Qi and Yin,blood stasis, dampness and damp - heat syndromes were domi- nant in diabetic nephropathy group. After dialysis the syndrome of Yin deficiency was more prominent, the syndrome of damp - heat was increased at first and then decreased, the syndrome of dampness was reduced significantly. The scores decreased gradually, especially syndrome of excess. Diabetic nephropathy group had obvious differences in distribution of syndrome compared with the non -diabetic nephropathy group. Conclusion：Diabetic nephropathy has special distribution regularity of syndrome. Syndrome of Yin deficiency plays an important role in diabetic nephropathy. And syndrome of damp - heat in diabetic nephropathy is more common than non - diabetic nephropathy. Dialysis can improve TCM syn- drome significantly.
