摘  要:目的:观察胸导管结扎和引流对重症腹腔感染大鼠急性肺损伤的影响。方法:40只雄性Wistar大鼠随机分为假手术(SO)组、腹腔感染模型(IAI)组、胸导管结扎(TL)组和胸导管结扎加引流(TLD)组,采用人工胃液联合大肠杆菌腹腔内注射制备大鼠腹腔感染模型,TL组和TLD组分别于制模后2 h行胸导管结扎和胸导管结扎并引流。各组大鼠分别于制模后6h观察肺和回肠组织病理改变和髓过氧化物酶(MPO)活性变化,取支气管肺泡灌洗液(BALF)行细胞学分析和肿瘤坏死因子-α(TNF-α)含量测定。结果:与模型组比较,TL组和TLD组大鼠肺组织病理损伤减轻,MPO活性降低,BALF的中性粒细胞百分比、TNF-α含量降低(P<0.05)。结论:阻断腹腔淋巴回流能减少腹腔感染大鼠肺组织中性粒细胞浸润和TNF-α释放,从而减轻肺损伤。Objective To investigate the in rats with severe intra-abdominal infection. effect of thoracic duct ligation and drainage on acute lung injury Methods Forty healthy male Wistar rats were randomly divided into 4 groups, namely sham operation (SO), intra-abdominal infection model(IAI), thoracic duct ligation (TL) and thoracic duct ligation and drainage(TLD). The rat model of severe intra-ahdominal infection was prepered by in- jecting E. coli intra-abdominally, the TL group underwent thoracic duct ligation at 2 h after model induction, and the TLD group was induced by ligating thoracic duct and draining it at the same time. Six hours after model induction, bronchoalveolar lavage fluid (BALF) of rats in each group was collected for cytological analysis andfor determination of tumor necrosis factor-α (TNF-al- pha), lung tissues were harvested for pathohistological evaluation and for the determination of myeloperoxi-dase (MPO) activity. Results Compared with model group, TL group and TLD group showed decreases in the pathohistological damage and MPO activity of lung, and in neutrophils and TNF-α of BALF (P 〈 0.05). Conclusion In rats with severe intra-abdominal infection,blocking the abdominal lymphatic flow can reduce neutrophil infiltration and TNF-α release in lungs and attenuate the pathohistological damage of lung.
