疟疾发热症候群的构建及价值研究

摘　　要：目的分析疟区发热病人的临床症状(体征),筛选与疟疾患病有意义的临床症状(体征),用以构建疟疾发热征候群。方法以《发热病人情况登记表》在疟疾传播期收集研究现场发热病人的临床症状(体征),对发热病人采血涂制标准血片进行镜检。计算机录入资料、建立数据库,以SPSS软件进行统计分析,比较各症状(体征)在疟疾病人和非疟疾发热病人中的分布有无差异,结合各症状(体征)作为诊断指标的灵敏度、特异度、阳性预测值和阴性预测值等进行筛选。结果共收集发热病人1 508例,其中疟疾病人241例。有10种症状(体征)在疟疾病人与非疟疾发热病人之间差异有统计学意义,分别是体温39℃及以上、周期性发作、少不规则发作、寒战、大汗、手脸苍白、肌肉关节酸痛、恶心厌食或呕吐、头痛、未出现腹泻。结论可使用这10种症状(体征)构建疟疾发热症候群用于早期筛查疟疾病人,指导疟疾临床诊断和治疗。Objective To analyze the clinical characteristics of fever patients and screen symptoms and signs significant tc diagnosis of malaria, so as to establish the fever related syndromes of malaria. Methods Information about symptoms and signs of fever patients was collected with the self - designed Registration Form for Patients with Fever. Blood was collected tc make standard films for microscopic examination. The data were statistically analyzed by the SPSS software. The distributions ot clinical symptoms and signs were compared between malaria patients and non - malaria patients. The symptoms and signs thai could serve as diagnostic indexes of malaria were screened based on their sensitivity, specialty, positive prediction values and nega- tive prediction values in malaris scfeening. Results Totally, 1,508 fever patients were surveyed, and 241 malaria patient, were found. Ten symptoms and signs distributed differently between malaria patients and non- malaria patients, that is, bod3 temperature above 39℃, periodic onset, rare irregular onset, chill, sweat, pale, muscle arthrosis pain, nausea or emesis, head- ache and no diarrhea. Conclusions Ten symptoms and signs can be used to establish fever related syndromes for early screer and clinical diagnosis and treatment of malaria.
