彩色M型多普勒超声评价煤尘肺病人的右室舒张功能

摘　　要：目的应用彩色M型多普勒超声心动图测量舒张早期右室三尖瓣口血流传播速度(Vp),评价煤尘肺患者的右室舒张功能。方法煤尘肺组病人89例(煤尘肺组),其中50岁以下21例,50～70岁53例,70岁以上15例;正常对照组90例,其中50岁以下21例,50～70岁52例,70岁以上17例。取心尖四腔或二腔心平面测量右室内血流Vp,绘制三尖瓣血流曲线。结果煤尘肺组不同年龄段的Vp值明显低于正常组(P均<0.05),且舒张早期右室内血流束远端形态异常呈圆钝状。2组三尖瓣血流曲线的E/A、Dec、DT值,均显示随年龄的变化而变化(P均<0.05),但2组间比较差异无统计学意义(P均>0.05)。结论彩色M型多普勒超声心动图应用于测量舒张早期右室内血流Vp,不受心脏负荷及年龄的影响,可以作为评价煤尘肺病人右室舒张功能的方法。Objective To evaluate right ventricular diastolic function in the patients with coal pneumoconiosis by color M-mode Doppler echocardiography for measuring flow propagation velocity（Vp） of tricuspid valve orifice in right ventric- ular early diastolic phase. Methods Eighty-nine patients with coal pneumoconiosis （aged less than 50 years in 21 cases, 50-70 years in 53 cases, more than 70 years in 15 cases） were selected as coal pneumoconiosis group, and 90 healthy sub- jects （aged less than 50 years in 21 cases, 50-70 years in 52 cases, more than 70 years in 17 cases） were selected as control group. The flow Vp in right ventricle in four or two chamber view was measured, and tricuspid valve blood flow curve was drawn. Results Compared with control group, all Vps at different agelsegment decreased in coal pneumoconiosis group（all P 〈 0.05）, and the distal blood flow beam of early diastolic phase in right ventricle showed abnormal dull and circular pat- tern. The values of peak E/A, deceleration rate （Dec） and deceleration time （DT） of tricuspid valve blood flow curve changed with the change of ages （ all P 〈 0.05 ） in two groups, but there was no difference between the two groups （ all P 〉 0.05 ）. Conclusions Because color M-mode Doppler echocardiography for measuring the Vp of early diastolic phase in right ventricle is not affected by cardiac load and age, it might serve as a method for evaluating right ventricular diastolic function of coal pneumoconiosis patients.
