摘 要:目的:观察丹参及丹参酮ⅡA磺酸钠(丹参酮Ⅱ钠)对大鼠脊髓缺血再灌注损伤脊髓细胞间黏附分子-1(ICAM-1)表达、白细胞介素-1β(IL-1β)及髓过氧化物酶(MPO)活性的影响,探讨其作用与机制。方法:SD大鼠随机分为手术对照组、损伤组、丹参组和丹参酮组。术前30min,前两组腹腔注射0.9%氯化钠溶液,丹参组给予丹参注射液,丹参酮组给予丹参酮Ⅱ钠。手术对照组打开腹腔但不夹闭腹主动脉,余3组制作脊髓缺血再灌注损伤模型。4组分别于术后0.5、1、4、8和12h检测脊髓组织中的ICAM-1、IL-1β及MPO。结果:手术对照组各观测时点未见阳性表达。其余3组伤后0.5h,IL-1β和ICAM-1开始升高,12h达到高峰值。丹参组与丹参酮组IL-1β和ICAM-1活性在4、8、12h较损伤组明显降低(P<0.05),8、12h差异显著(P<0.05,P<0.01)。伤后4h开始出现ICAM-1阳性表达血管,12h内持续增多,其中丹参组与丹参酮组ICAM-1阳性表达血管在4、8、12h时较损伤组明显减少(P<0.05)。丹参酮组在8、12h时点尤其显著(P<0.05,P<0.01)。3组MPO于0.5h后开始上升,并在12h达到高峰值,丹参组与丹参酮组在4h、8h、12h较损伤组明显降低(P<0.05)。结论:丹参及丹参酮Ⅱ钠可以降低脊髓缺血再灌注损伤中脊髓的IL-1β活性及ICAM-1的表达,减少中性粒细胞的浸润,减轻炎症反应。丹参酮Ⅱ钠的作用优于丹参。Objective:To detect the expression of intercellular adhesion molecule-1、the activity of interleukin-1β and myeloperoxidase during Spinal Cord Ischemic/Reperfusion injury via Tanshinone-IIA in order for its involved mechanism.Methods:Sprague Dawley rats were randomly divided into four groups:sham-operated,ischemia,Danshen and Tanshinone-IIA groups(n=35).Abdomianal aorta occlusion was performed along the right renal arterial root to induce spinal cord ischemia.Blood flow was recoverd 30 minutes following occlusion to establish models of spinal cord ischemia/referfusion injury.Abdominal aorta occlusion was not performed in the sham operation group.Thirty minutes prior to ischemia,rats of the Danshen group were intraperitoneally injected with Danshen and rats of the Tanshinone-IIA group were intraperitoneally injected with sulfo Tanshinone-IIA.The expression of ICAM-1 and the activity of IL-1β,MPO were observed at hour 0.5h,1h,4h,8h,12h following perfusion.Results:There was no significantly changes in the sham-operated group.All rats in ischemia,Danshen and Tanshinone-IIA group,IL-1β together with ICAM-1 increased at 0.5h after reperfusion,and keep increasing gradually following 12 hours.The activity of IL-1β and ICAM-1in the Danshen and Tanshinone-IIA group was significantly reduced than sham-operated group at 4h,8h and 12h after reperfusion(P〈0.05).It significantly decreased in the Tanshinone-IIA group than the Danshen group at 8h and 12h after reperfusion(P〈0.05,P0.01).There were ICAM-1-positive blood vessels 4hours after reperfusion,and keep increasing till reperfusion 12 hours in either Danshen or Tanshinone-IIA group and significantly reduced than sham-operated group at 4,8 and 12h after reperfusion(P〈0.05).ICAM-1 of the Tanshinone-IIA group was especially less than Danshen group at 8 and 12h after reperfusion(P〈0.05,P0.01).MPO were increased at 0.5h after reperfusion,and keep increasing in 12h in Danshen and Tanshinone-IIA group.Conclusions:Spinal cord ischemia/reper
