364例急性心肌梗死中医证型分布规律研究

摘　　要：目的:对364例急性心肌梗死患者中医证型分布进行回顾性调查,为临床辨证治疗提供参考。方法:将364例急性心肌梗死患者按ST段抬高型心肌梗死和非ST段抬高型心肌梗死分为两组,对原临床辨证的中医证型进行归纳,计算同一证型在两组间的构成比。结果:364例患者中医辨证共分7型,气虚血瘀、气虚痰瘀互阻、气虚痰浊闭阻属于虚实夹杂证,痰瘀互阻、寒凝心脉属于实证,气阴两虚、阴竭阳脱属于虚证。其中ST段抬高型心肌梗死组(263例)虚实夹杂证占33.46%,实证占55.51%,虚证占11.03%;非ST段抬高型心肌梗死组(101例)虚实夹杂证占69.31%,实证占20.79%,虚证占9.90%。结论:急性心肌梗死中,ST段抬高型心肌梗死实证比例较高,非ST段抬高型心肌梗死虚实夹杂证比例较高,而虚证两组间无明显差异。Objective：To analysis the pattern type distribution of 364 patients with acute myocardial infarction retrospectively and to provide a reference for the clinical treatment.Methods：364 patients with acute myocardial infarction were divided into two groups：ST-segment elevation myocardial infarction and non-ST-segment elevation myocardialinfarction.Summarize the clinical TCM pattern type and calculate the constituent ratio of the same pattern type between the two groups.Results：364 patients were divided into 7 TCM pattern types：qi deficiency and blood stasis,qi deficiency with phlegm and blood stasis,qi deficiency with phlegm stasis,phlegm and blood stasis,cold congealing in the heart vessel,Qi and Yin vacuity,exhaustion of yin and desertion of yang.The vacuity-repletion complex pattern was 33.46%,the repletion pattern was 55.51%,and the vacuity pattern was 11.03% in ST-segment elevation myocardial infarction group（263 cases）.In non-ST-segment elevation myocardial infarction group（101 cases）,the proportion is respectively 69.31%,20.79% and 9.90%.Conclusion：ST segment elevation myocardial infarction group was superior with the repletion pattern.Non ST segment elevation myocardial infarction group was superior with the vacuity-repletion complex pattern.There was no significant difference between the two groups with the vacuity pattern.
