摘 要:目的 探讨股骨质量对非骨水泥假体置换近期疗效的影响以及皮质骨指数 (CI)对股骨质量的评价意义 ,以指导临床假体选择和术后康复。方法 49名非骨水泥假体置换患者进行平均 3 5年的随访 ,42名患者有术前或术后当天X线片。结果 皮质骨指数低者 ,Harris评分及病人满意度较低 (P <0 0 5 ) ,大腿处疼痛率较高 (P <0 0 5 ) ,且疼痛持续时间长。结论 皮质骨指数是临床上评价股骨质量的可靠半定量指标。骨质疏松患者易于发生大腿处疼痛。CI≤ 2 2者 ,应延缓负重时间 ,减少产生扭力矩的动作 。Objective To study the clinical influence of femoral quality on noncemented hip replacement and to evaluate cortical index(CI)for femoral quality in order to guide prosthesis choose and rehabilitation.Methods One hundred and twenty-one cases of noncemented hip replacement were followed for average 3 5 years,42 of which had x-ray of preoperation or operation day.Results Harris scores and patients' satisfaction were lower but pain was more significant and prolonged when CI were lower.Conclusions CI was a reliable semi-quantity parameter for clinical evaluation of femoral quality.Osteoporosis patients predisposed thigh pain.Lag for full-weight loading and avoidance for torsional motion contributed to lessen pain when CI≤2 2.
