摘 要:目的研究大鼠重症急性胰腺炎(SAP)肾组织细胞间凋亡抑制因子Survivin表达与肾损伤的关系。方法 40只SD大鼠,体重(250±20)g,随机分为假手术组和重症急性胰腺炎6h、12 h、24 h组,每组10只。实验组采用胰胆管逆行穿刺注射4.5%牛磺胆酸钠(1.0 ml/kg)建立不同炎症程度的SAP模型,比较假手术组与各实验组胰腺组织和肾组织病理学变化,检查各组血清淀粉酶(AMY)、血肌酐(Cr)和血尿素氮(BUN),应用免疫组化方法检测胰腺和肾组织中Survivin的表达,肾脏组织细胞凋亡测定法(TUNEL法)检测肾组织细胞凋亡的变化。结果各实验组AMY、Cr、BUN较假手术组有所升高,Survivin在假手术组胰腺组织和肾组织表达均呈阴性,Survivin在SAP大鼠胰腺组织中的表达主要集中在胰腺导管处,腺泡细胞也有散在表达,Survivin在SAP大鼠肾组织中表达主要集中在肾小管和肾小球。肾小球细胞凋亡指数明显高于对照组[6、12、24 h分别为(3.10±1.00)vs(0.79±0.11),P<0.05;(4.01±1.90)vs(0.87±0.12),P<0.05;(5.00±1.80)vs(0.81±0.19),P<0.01],肾小管细胞凋亡指数明显高于对照组[6、12、24 h分别为(9.50±1.01)vs(1.29±0.47),P<0.01;(11.90±2.30)vs(1.27±0.29),P<0.01;(14.9±2.57)vs(1.21±0.32),P<0.01],且随SAP病程延长,肾组织凋亡细胞水平逐渐升高。结论 Survivin在SAP大鼠胰腺组织中及并发肾损伤的肾组织中呈异常高表达,Survivin表达的高低与肾组织损伤的严重程度有关,可能起到减轻肾损伤、促进其修复的作用。Objective To investigate the expression of Survivin in kidney tissues during severe acute pancreatitis (SAP) complicated with re- nal injury using a rat model. Methods Sprague -Dawley rats (weight: 230 -270 g) were randomly divided into four groups (n = 10 each) : control: sham operation; SAP induction: 6 h, 12 h, and 24 h post -infusion. Rats in the SAP groups were given 4.5% sodium taurocholate injection into the biliopancreatic duct. Severity of SAP complicated with renal injury was analyzed by detecting changes in serum amylase (AMY), creatine (Cr), blood urea nitrogen (BUN), and histology of pancreas and kidney. Changes in Survivin protein expression were de- tected by immunohistochemistry. Changes in the apoptotic ratio of acinar cells in pancreas and kidney was determined by terminal deoxynucle- otidyl transferase dUTP nick end labeling assay. Results Serum AMY, Cr, and BUN were higher in SAP groups (vs sham operated group). Survivin expression was negative in the sham operated group, but in the SAP groups was positive in both pancreatic tissue, mainly in duetal cells and a few acinar cells, and renal tissue, mainly in tubules and glomeruli. The apoptotic index of renal glomerular cells showed a time - dependent significant increase in SAP groups, compared to the sham - operated group [ 6 h : (3.10 _± 1.00) vs (0. 79 ± 0.11 ), P 〈 0.05 ; 12 h : (4.01 ±1.90) vs (0.87 ±0.12), P〈0.05; 24 h: (5.00 ±1.80) vs (0.81 ±0.19), P 〈0.01], as did the apoptotic index of renal tubule cells [6 h: (9.50±1.01) vs (1.29±0.47), P〈0.01; (11.90±2.30) vs (1.27 ±0.29), P〈0.01; (14.9 ±2.57) vs (1.21 ±0.32), P 〈 0. 01 ]. In general, the amount of apoptotic cells gradually increased over time in renal tissues of SAP groups.
