江苏省暗娼性病新发感染及失访人群特征分析

摘　　要：目的了解江苏省暗娼人群艾滋病毒(HIV)和梅毒(TP)新发感染及队列研究中失访人群的特征。方法采用方便抽样建立暗娼队列,3个月后进行随访。在基线和随访时对每个调查对象进行匿名问卷调查,对随访和失访对象的人口学、行为学和实验室指标进行比较。抽取5 ml静脉血和2支宫颈拭子检测性病(STI)、HIV。结果本次共调查1 806名暗娼,3个月后随访到737名,随访率为40.8%,HIV和TP血清阳转率分别为0.0/100人年和6.1/100人年;接受的教育程度高、来源的场所档次高、最近一年在本地未接受过HIV宣传服务及TP感染者更容易失访。结论虽然江苏省暗娼人群处于HIV低流行状态,但是TP新发感染率高,失访暗娼感染STI、HIV的危险性高,应更加关注高档场所暗娼的干预工作。Objective To investigate the incidence rates of human immunodeficiency virus （HIV） and syphilis and analyse the characteristic of female sex workers who lost to follow-up in cohort study. Methods Convenience sampling was used to recruit the subjects. Demographic and behavioral information was obtained in terms of face to face questionnaire interview. Blood sample and cervical swab were collected for the tests of HIV, treponema pallidum （TP）, chlamydia trachomatis （CT） and neisseria gonorrhoeae （NG）. The same investigation was conducted 3 months later. Results A total of 1 806 subjects were recruited with questionnaire information and blood samples was conducted. The rate of retention in cohort was 40. 8% （737/1 806）. During the 3-month follow-up period, HIV and TP incidence appeared to be 0.0 per 100 person-year and 6. 1 per 100 person-year respectively. The subjects receiving higher education, working in the high class venues, being not covered by any HIV educations, and being infections of TP were apt to fail in follow-up. Conclusion High STI incidence rates and widespread risk behaviors are identified among female sex workers in Jiangsu province although HIV prevalence is still relatively low. Drop-out cases are apt to infect TP and HIV, and more work should be done about FSW especially on those working in high class venues to prevent them from failing to follow-up.
