摘  要:探讨磁共振(MRI)三维梯度回波磁敏感成像(three-dimensional gradient recalled echo susceptibility weighted imaging,3D-GRESWI)对颅内海绵状血管瘤(intracranial cavernous angioma,ICA)的诊断价值。方法 收集18例患者均进行常规MRI序列及SWI扫描,其中13例随后进行CT检查。对所获得的SWI原始图像进行12mm层厚的最小密度投影重组。然后将SWI重组图像、常规MRI图像及CT图像就病灶大小、形态、信号特点、强化情况作比较分析。结果 常规MRI发现33个病灶,病灶直径约0.6~2.8cm,平均约1.62cm;SWI发现134个病灶,病灶直径约0.2~2.8cm,平均约1.43cm,且可见23个直径〉1.2cm病灶的引流静脉;CT发现12个病灶,漏诊1例,病灶直径约1.2~2.7cm,平均约1.83cm。结论 SWI对ICA十分敏感,能发现CT及常规MRI未能发现的病灶,对ICA的诊断和治疗具有重要意义。Objective To investigate the value of susceptibility weighted imaging(SWI) in diagnosis of intracranial cavernous angioma(ICA). Methods 18 cases (10 men and 8 women,mean age 35.2 years) with ICA were collected. All cases underwent MR imaging, including SWI sequence include conventional sequences(SE T1,TSE-T2,FLAIR) and SWI . Then 13 cases, of them, 13 cases underwent CT examinations simultaneously. The images of SW Iwere reconstructed by minimumintensity projection at 12 mm thickness. The size , morphous,signal (or intensity) and contrast-enhanced condition of the foci were compaired on SWI images, conventional MRI and CT images. Results 33 foci were detected by conventional MRI, the diameter of the foci was 0.6-2.8 cm, mean 1.62 cm. Meanwhile, 134 foci were showed by SWI, the diameter of the loci was 0.2-2.8 cm, mean 1.43 cm,and the draining veins in 23 loci with diameter over 1.2 cm were also found. 12 foci with diameter of 1.2-2.7 cm, mean 1.83 cm were showed by CT, but one case was missed. Conclusion SWI is very sensitive in diagnosis of ICA and can show the loci which can not detect by CT and conventional MRI, SWI is very significant in the diagnosis and treatment of ICA .
