摘 要:目的 观察部分液体通气 (PLV)对急性肺损伤 (ALI)家兔肺内气体交换、肺顺应性、体循环功能的影响。方法 健康雄性新西兰兔 2 4只 ,用油酸制备成急性肺损伤模型后随机分为 3组 ,每组 8只。各组采用不同方法治疗 :呼气末正压 (PEEP)组 ,常规机械通气 (CMV) +PEEP治疗 ;生理盐水(NS)组 ,肺内注入NS同时 +CMV +PEEP ;FDC组 ,肺内注入全氟碳化合物 全氟萘烷 (FDC)同时 +CMV +PEEP。分别观察各组治疗前后血气、呼吸力学、体循环的变化。结果 FDC组动脉血氧分压从(90± 6 )mmHg升高到 (2 32± 2 6 )mmHg(P <0 0 5 ) ,动脉血二氧化碳分压从 (5 2 1± 2 8)mmHg降低到(40 1± 3 0 )mmHg(P <0 0 5 ) ,肺顺应性从 (1 0 5± 0 11)ml/cmH2 O升高到 (1 79± 0 2 4)ml/cmH2 O(P <0 0 5 ) ,血气指标及肺顺应性的改善优于其它两组。结论 PLV可明显改善ALI家兔的动脉血气 ,有效提高肺顺应性 ,对动物的体循环无明显影响 ,疗效优于CMV。Objective To observe the effectiveness of partial liquid ventilation (PLV) on gas exchange, pulmonary compliance (Cst), blood dynamics in the oleic acid (OA) induced acute lung injury (ALI) rabbits Methods 24 healthy male New Zealand rabbits were randomly allocated to three groups and treated with various methods after OA acute lung injury model established. The study groups as follows: PEEP group with conventional mechanical ventilation (CMV) and PEEP, normal saline (NS) group with CMV, PEEP and NS, FDC group with CMV, PEEP and FDC The parameters such as blood gases, Cst, circulation state were observed before and after treatment Results In FDC group, PaO 2 increased from (90±6) mm Hg to (232±26) mm Hg ( P <0 05), PaCO 2 decreased from (52 1±2 8) mm Hg to (40 1±3 0) mm Hg ( P <0 05), Cst increased from (1 05±0 11) ml/cm H 2O to (1 79±0 24) ml/cm H 2O( P <0 05). HR, MAP and CVP showed no significant changes after treatment compared with pretreatment values in FDC group Conclusion Pulmonary gas exchange and pulmonary compliance improved significantly during PLV in the ALI rabbits, without obvious influence on hemodynamic status.
