摘  要:目的评价慢性阻塞性肺疾病(COPD)患者应用出院计划服务的效果。方法将100例COPD患者分为试验组和对照组各50例,试验组实施出院计划服务,对照组接受常规护理和随访。分别在出院后1、3、6个月进行西雅图阻塞性肺疾病问卷(SOLDQ)的测量。结果干预期间两组患者住院天数、住院总费用的差异无统计学意义(P>0.05);干预后疾病加重次数、再入院次数及自我照顾认知总分的差异有统计学意义(P<0.05);试验组SOLDQ的生理功能、情绪功能、照护满意度的得分高于对照组(P<0.05),应对技能得分差异无统计学意义(P>0.05)。结论出院计划服务可以提升COPD患者自我照护的认知能力,减少疾病加重次数及再入院,提高生活质量。Objective To evaluate the effect of discharge planning services in patients with chronic obstructive pulmonary disease(COPD). Methods One hundred COPD patients were divided into two groups. The patients in the experimental group received discharge planning services while the control group received routine follow-up services. The quality of life were measured by Seattle Obstructive Lung Disease Questionnaire(SOLDQ) at 1,3,and 6 month after discharge. Results There were significant differences in the frequency of acute exacerbation,incidence of hospital readmission and total score of self care between the two groups(P〈0.05);no significant difference was found in the duration of hospital stay and cost of hospitalization (P〉0.05). The scores of physical function,emotional function and satisfaction with self care in the experimental group were significantly higher than those of the control group(P〈0.05),while no significant difference was found in the score of coping skills(P〉0.05). Conclusion Discharge planning services can enhance the ability of self-care,reduce the frequency of acute exacerbation and hospital readmission,and improve the quality of life in patients with COPD.
