从叶天士“久病入络”思想辨治类风湿关节炎痰瘀痹阻证

摘　　要：叶天士的"久病入络"学术思想,其理论揭示了一般疾病发展的共同规律和邪气入深的共同通路,并指出其主要病理机转为络中气滞,血瘀或痰阻,在治疗上主用清络宣通、虫蚁搜剔之法,形成了系统的"久病入络"体系。笔者根据多年临证,体会到痹症日久迁延不愈,痰浊、瘀浊相互搏结,蕴结成毒,浊毒流注全身,导致恶性循环。故在从毒论治痹病的基础上,宗叶氏之法,以涤痰祛瘀,搜剔经络,解毒排毒为治法,运用涤痰蠲痹汤加减治疗类风湿关节炎痰瘀痹阻证。Ye Tianshi＇s academic thought of ＇Jiu Bing Ru Luo＇,which theory reveals the general disease law of development and the common pathway of pathogens entering the interior,and pointed out the main pathogenesis is collaterals stagnation,blood stasis and phlegm.therapeutically the main idea is clearing collateral ventilation,expelling phlegm and regulating meridian by insect drugs.All of them finally form the system of ＇Jiu Bing Ru Luo＇.Based on years of clinical,We have realized a long protracted course of arthromyodynia made phlegm,blood stasis interacted,yet formed an toxin,turbid poison drop body,lead to a vicious circle.So on the basis of toxin thougt of arthromyodynia treatment,According to Ye Tianshi＇s theory,We have made treatment method that eliminating phlegm and remove blood stasis,regulating meridian,detoxicating and eliminating of toxicant,and used Di Tan Juan Bitang in treatment of rheumatoid arthritis of type of obstruction of phlegm and blood stasis.
