丙泊酚辅助下无创正压通气治疗急性左心衰竭的疗效分析

摘　　要：目的探讨丙泊酚辅助下无创正压通气治疗急性左心衰竭的临床疗效。方法回顾分析南方医科大学附属小榄人民医院ICU近4年75例急性左心衰竭患者的临床资料,其中药物组(A组)19例,药物+无创正压通气组(B组)28例,药物+丙泊酚+无创正压通气组(C组)28例,对比3组患者心力衰竭缓解率、心力衰竭缓解平均时间、改有创机械通气率及治疗过程中误吸率。结果 C组心力衰竭缓解率、心力衰竭缓解平均时间均明显优于B组和A组,改机械通气率低于A组和B组,差异有统计学意义(P<0.05),3组误吸率的差异无统计学意义(P>0.05)。结论丙泊酚辅助下无创正压通气是治疗急性左心衰竭的有效方法,可避免机械通气带来的不良作用,值得临床实践应用。Objective To discuss the clinical effect of non-invasive positive pressure ventilation assisted by Propofol in the treatment of acute left heart failure. Methods The clinical data of 75 patients with acute left heart failure of ICU of Xiaolan People＇s Hospital Affiliated to Southern Medical University in the lately 4 years were analyzed retrospectively. There were 19 cases of drug group （group A）, 28 cases of drug ＋ non-invasive positive pressure ventilation group （group B）, and 28 cases of drug ＋ Propofol ＋ non-invasive positive pressure ventilation group （group C）. The remission rate of heart failure, average remission time of heart failure, the rate of changing into invasive mechanical ventilation and the aspiration rate in the treatment process. Results The remission rate of heart failure and average remission time of heart failure in group C were significantly better than group A and B, and the rate of changing into invasive mechanical ventilation was lower than group A and B, the differences were statistically significant （P 〈 0.05）. There were no statistically significant differences of the aspiration rate among the three groups （P 〉 0.05）. Conclusion Non-invasive positive pressure ventilation assisted by Propofol is an effective method in the treatment of acute left heart failure, which can avoid the side effects of invasive mechanical ventilation, and is worthy of clinical application.
