摘  要:目的 :研究心肌肌钙蛋白 T(cardiac troponin T,c Tn T)在心肌炎中的动态变化及与心功能的相关性。方法 :应用夹心层免疫吸附法测定 30例心肌炎 ,2 0例疑似心肌炎及 30例正常对照 c Tn T和肌酸磷酸激酶 (creatinekinase,CK)及同功酶 CK - MB,同步进行心功能测定。结果 :心肌炎组 c Tn T明显高于疑似心肌炎组及对照组 ,且心肌炎组治疗前明显高于治疗后 ,疑似心肌炎组与对照组无显著差异。 c Tn T与心脏收缩功能 (CI,EF,ACT)均呈负相关。 c Tn T的敏感性为 76 .6 7% ,特异性为 96 .6 7%。结论 :c Tn T对心肌损害的敏感性及特异性均高于 CK和 CK-MB,其动态变化提示心肌病变的轻重及预后。 c Tn T与心脏收缩功能呈负相关 ,即 c Tn T越高 ,收缩功能越差。Objective:To observe the dynamic variety of cardiac troponin T(cTnT) and the interrelation between the changes in cTnT and cardiac systolic function in myocarditis. Methods:In this stydy concentration of serum cardiac troponin T was measured by sandwich-immunoassay in 30 patients with myocarditis(group Ⅰ), 20 children with suspected myocarditis(group Ⅱ), and 30 healthy children(control) and were compared with creature kinase CK and isoform-MB, meanwhile myocardial systolic function (CI,EF,ACT) were detected in group Ⅰ before and after treatmen. Results:In group Ⅰ the cTnT level was eleveated at the beginning of treatment the positive rate of cTnT was 76.67%, which was higher than that of CK-MB(36.67%) and CPK(30%) in group Ⅰ. The sensitivity of cTnT was 76.67% while the specificity was 96.67%; The interrelations between the cTnT and CI,EF,ACT were negative in group Ⅰ. Conclusion:cTnT was more sensitive and special marker for myocarditis than CK and CK-MB; the dynamic variety of cTnT may indicate the degree and prognosis of myocarditis; the more cTnT concentration the worse cardiac systolic function was.
