摘 要:目的 为改善脑性瘫痪患儿的预后 ,研究围产期脑损伤儿早期诊断脑瘫的方法。方法 在临床神经发育学检查的基础上 ,检测正中神经体感诱发电位。以月龄分组 ,正常儿做对照 ,判断异常指标。结果 临床筛查出的 84例脑瘫高危儿 ,经体感诱发电位检测异常结果的患者共 60例 ( 71 43 % )。远期随访证实 60例中符合脑瘫诊断标准的 5 3例。其余 2 4例正常结果中脑损伤儿只有 2例为轻型脑瘫。真实发病率为 65 48% ,准确性为 89 2 9% ,敏感性为 96 3 6% ,特异性为 75 86%。结论 围产期脑损伤儿体感诱发电位检测对早期诊断脑瘫是一种客观、灵敏、有效的实验室方法。Objective For better prognosis of cerebral palsy children, to research the method of cerebral palsy earlier diagnosis. Methods Median nerve somatosensory evoked potentials (MNSSEP) were examined on the base of infants nerve development examination. According to the outcomes of normal infants on every month, to judge abnormal standard. Results In 84 higher dangerous cerebral palsy infants by clinial screen, 60 cases have abnormal results about examinating MNSSEP ( 71.43 %).Distant visiting proves: there are 53 cases according with the diagnosis standards of cerebral palsy in the 60 cases. But only 2 cases developed to light cerebral palsy in the other 24 cases with normal MNSSEP. The real incidence was 65.78 %, the accuracy 89.29 %, the sensitivity 96.36 %, the specificity 75.86 %. Conclusion The somatosensory evoked potentials in perinatal cerebral injury infants is an objective, sensitive and effective laboratry assistance method in earlier diagnosis of cerebral palsy.
