摘  要:目的观察。肾病综合征出血热(HFRS)病程中形态学改变与血流动力学改变的规律。方法用彩色多普勒超声仪对198例经血清学证实为HFRS患者病程中。肾内动脉的血流频谱及血流量与150例正常人对照。形态学通过二维声像图观察肾脏体积、肾实质厚度、椎体回声的改变。结果发热期、恢复期各级肾动脉血流频谱各项参数与正常对照组比较,均未见统计学差异(P〉0.05)。低血压休克期、少尿期、多尿期各级肾动脉阻力指数明显高于正常组(P〈0.01),收缩期峰值流速、舒张期末血流速度在低血压休克期、少尿期、多尿期低于对照组(P〈0.05)。低血压休克期、少尿期、多尿期肾脏形态学改变与正常组比较体积明显增大、肾实质明显增厚、椎体回声减低,部分可见肾包膜下出血。结论HFRS病程中各期形态学和各级肾血流动力学改变存在着规律性变化,形态学与肾血流动力学变化相结合动态观察为临床医师提供了解肾脏病情进展情况,可检测治疗效果,对评估预后发展提供了重要依据。Objective Observation of hemorrhagic fever with renal syndrome ( HFRS ) in the course of the morphological changes and hemodynamic changes. Methods By color Doppler ultra- sound in 198 patients with serologically confirmed HFRS in patients with acute renal artery blood flow and blood flow spectrum and 150 normal controls. Morphological observation of kidney volume by two-dimensional ultrasound, renal parenchymal thickness, vertebral echo change. Results Of fever, recovery at all levels of renal artery blood flow spectrum of the parameters and the normal control group showed no significant difference (P〉0. 05). Of hypotension, shock, oliguria, polyuria levels of renal artery RI was significantly higher than the normal group(P〈0.01), Vd, Vs in the hypotension period, oliguria, polyuria lower than the control group (P〈0. 05). Of hypotension, oliguria, polyuria of renal morphological changes in the normal group size was significantly increased, marked thickening of the renal parenchyma, decreased vertebral body echo part saw bleeding under the renal capsule. Conclusions HFRS course of the disease there are regular changes in the morphology and the levels of renal hemodynamic changes in the combination of dynamic morphology and renal hemodynamic changes observed for the clinician to provide understanding of kidney disease progression, can detect the treat- ment effect, the assessment the prognosis of the development provides an important basis.
