妇产科腹腔镜手术38例并发症的处理对策

摘　　要：目的:探讨妇产科腹腔镜手术并发症及其处理对策。方法:选取近5年来我院妇产科腹腔镜手术患者1073例,回顾分析其中38例发生并发症患者的临床资料并进行总结。请所有患者评价治疗满意度,对评价结果进行分析。结果:1073例行腹腔镜手术的妇产科患者38例出现手术并发症,发生率3.54%。其中皮下气肿12例(31.58%),肩背酸胀10例(26.32%),穿刺孔渗血7例(18.42%),出血5例(13.16%),术后感染3例(7.89%),输尿管损伤1例(2.63%)。所有患者经过处理后均痊愈。我们发现发生并发症的患者对治疗表示非常满意者仅3例,占7.89%,明显低于未发生并发症患者98.36%的比例,两组比较差异明显,有统计学意义(P<0.05);但是总体满意度两组差别不大(P>0.05)。结论:在进行腹腔镜手术时应充分考虑各个环节,防止并发症发生,一旦发生并发症应及时采取相应处理,最终达到手术的安全和患者生活质量的提高。Objective： To observe the complications of laparoscopic operation and the countermeasures in obstetrics and gynecology department. Method： 1073 patients treated with obstetrics and gynecology lapa- roscopic operation in our hospital in recent five years were selected, and the clinical data of 38 patients with complications among them were analyzed retrospectively and summarized. Ask all the patients to evaluate the treatment satisfaction, and analyze the evaluation result. Results： Among the 1073 patients, there were 38 cases appearing operation complications with the occurrence rate of 3.54%. There were 12 cases of subcuta- neous emphysema（31.58% ）, shoulder soreness （26.32%）, puncture bleeding in 7 cases{ 18.42% ）, 5 cases of hemorrhage（ 13.16% }, 3 cases of postoperative infection（7.89% }, and ureteral injury in 1 case （2.63%）. All the patients were cured after treatment. 89% ） , which were lower significantly than the patients There were only 3 patients with complications （7. without complications （98.36%）. There were sig- nificant difference between the 2 groups, which had statistical significance （ P〈0.05 ）. But there was no great difference in overall satisfaction { P〉0.05 ）. Conclusion： In sideration to all aspects, prevent the complications and take happened to ultimately achieve the operation safety and the laparoscopic operation, we should give full con- corresponding processing once the complications improvement of life quality in patients.
