皮肤原位再生复原医疗技术在磷烧伤创面修复中的临床应用

摘　　要：磷烧伤是热力和化学物质的复合烧伤,通常创面较深,严重者可达骨骼本院烧伤科2006年1月至2010年12月共收治磷烧伤住院患者248例,均采用皮肤原位再生复原医疗技术(MEBT/MEBO)治疗,创面愈合好,所有患者均无功能障碍,临床效果满意治疗要点如下:1.清除创面上的磷颗粒,避免与空气接触燃烧;2.在第1个24h内,每2h换药1次,以尽快排除创面残留的磷:3.次日,进行药刀结合,耕耘、搔刮创面,每6h清理创面更换MEBO药膏1次,在去除残留药膏时再配合搔刮、耕耘创面,每日1次,连续3d;4.伴有吸入性损伤者,现呼吸困难或肺水肿时,尽早行气管插管或切开;5.严格按照MEBT/MEBO规范进行系统治疗和局部换药。Phosphorous burns is a kind of composite thermal and chemical burns,the wound is generally much deep and may down to bone in serere case.A total of 248 cases of phosphorus burns were enrolled into our borns department between Jan.2006 and Dec.2010,and all were treated with MEBO/MEBO.The clinical therapeutic efficacy was good in that all the wounds were healed and no patient developed dysfunction.The key points of treatmen are as follows：1.Remove the phosphorus granules to avoid burning upon contact with air;2.During the first 24 hours,change dressing every two hours so as to remove the residual phosphorus as soon as phossible;3.On the second day,use MEBO combined with surgical methods including incising and scraping the wounds,clean the wound and change MEBO erery 6hrs.During the re-dressing,remove the remnant MEBO and scrape and incise the wound once daily for three days;4.For the cases of inhalation injury and complicated with dyspnea or pulmonary edema,perform trachea cannula or tracheotomy timely;5.Strictly follow the standard procedures of MEBT/MEBO to perform systemic treatment and topical dressing change.
