超声检测PCOS不孕症患者腹腔内脂肪的研究

摘　　要：目的探讨经超声检测内脏脂肪厚度在多囊卵巢综合征(PCOS)诊疗中的价值,为更好地治疗PCOS不孕症提供依据。方法采用超声检测199例育龄期PCOS及87例非PCOS患者的肝前脂肪、肝前皮下脂肪、内脏脂肪、腹部皮下脂肪,并测量身高、体重、腰围、臀围等,对两组间上述指标进行对比分析。结果 PCOS组肝前脂肪厚度、内脏脂肪厚度均大于非PCOS组(P<0.05);PCOS组的体重、体质量指数(BMI)、腰围、腹围、臀围、腰臀比(WHR)均明显高于非PCOS组(P<0.05);身高则差异无统计学意义(P>0.05)。结论 PCOS不孕症患者腹腔内脂肪及腹部皮下脂肪厚度值均明显高于非PCOS不孕症患者,减重尤其是减少内脏脂肪堆积对治疗PCOS具有重要意义。Objective To explore the value of prehepatic fat thickness measured by ultrasonography in the diagnosis and treatment of polycystic ovary syndrome（PCOS）,and to provide more references for the treatment of infertility patients with PCOS.Methods One hundred and ninty-nine PCOS cases of childbearing age（PCOS group） and 87 non-PCOS cases（non-POCS group） were enrolled in the study.Their prehepatic fat thickness,prehepatic subcutaneous fat thickness,visceral fat thickness and abdominal subcutaneous fat thickness were detected by ultrasound,and their height,weight,waist circumference and hip circumference were also measured.A comparison of the indices above was done between the two groups.Results Prehepatic fat thickness and visceral fat thickness of PCOS group were significantly larger than those of non-PCOS group（P〈0.05）;Weight,body mass index（BMI）,waist circumference,abdominal circumference,hip circumference and waist-to-hip ratio of PCOS group were significantly higher than those of non-PCOS group（P〈0.05）;No significant difference was found in height between the two group（P〉0.05）.Conclusion Intraabdominal adiposity and abdominal subcutaneous fat thickness of PCOS patients are significantly higher than those of non-PCOS patients.Weight-reducing,especially reducing visceral fat is of great importance to the treatment of PCOS.
