老年人重度心衰的临床特点

摘　　要：目的探讨老年重度心衰的临床特点。方法回顾性分析老年心功能Ⅲ级、Ⅳ级患者119例,分为60～69岁组、70～79岁组、≥80岁组3组,分别从引起心衰的慢性基础疾病比较、心衰合并症比较及引起心衰发作或加重诱因等方面进行分析。结果随年龄增长,致心衰基础疾病以冠心病、高血压病、慢性阻塞性肺病、尿毒症为主,而风湿性心瓣膜病作为致心衰基础疾病者在70岁以下患者多见(P<0.005)。引起心衰诱因以肺部感染及快速房颤等心律失常多见。结论冠心病、高血压病、慢性阻塞性肺病是引起老年人心衰的主要基础疾病,而肺部感染、快速房颤、房扑是导致老年人心衰加重的主要诱因。临床工作中应注意对冠心病的防治、控制血压,积极预防及控制慢性阻塞性肺病的发生与进展,降低老年心衰的发生率、死亡率。Objective To explore the clinical characteristic of the aged patients with advanced heart failure. Methods 119aged patients with advanced heart failure were divided into three groups. The first group age was from 60 to 69 years,the secondgroup was from 70 to 79 years,the third group was the aged more than 80 years. The datas that chronic diseases which caused heartfailure、complications of congestive heart failure、heart failure exacerbation factors were analysised. Results With the growth of age,the chronic diseases which caused heart failure were the coronary heart diseases、hypertension、chronic obstructive pulmonary disea-ses、uremia. The chronic diseases which caused the heart failure of 70 years was rheumatic heart valve disease. The factors whichcaused heart failure exacerbation were pulmonary infection、rapid atrial fibrillation. Conclusion The chronic diseases which causedthe heart failure of over 70 years were coronary heart diseases、hypertension、chronic obstructive pulmonary diseases. The factorswhich caused heart failure over 70 years exacerbation were pulmonary infection、rapid atrial fibrillation、atrial flutter. In our clinicalwork,we must control and prevent these factors to reduce incidence rate and mortality rate of over 70 years.
