摘　　要：目的探讨不同体质指数(BMI)的2型糖尿病患者(T2DM)的可溶性血管细胞黏附分子1(SVCAM-1)水平以及胰岛素抵抗(IR)程度。方法将130例T2DM患者根据BMI分为正常体重组(41例)、超重组(49例)和肥胖组(40例),比较各组的一般情况、血脂、糖代谢等生化指标和SVCAM-1、IR程度的差异。结果(1)肥胖组的SVCAM-1水平高于正常体重组NMS组(P<0.05),肥胖组与超重组FIns、HOMA-IR、IAI高于体重正常组(P<0.05)。(2)相关分析显示,血清SVCAM-1与收缩压(SBP)、空腹血糖(FPG)、三酰甘油(TG)、胰岛素抵抗指数(HOMA-IR)、胰岛素作用指数(IAI)正相关,与高密度脂蛋白(HDL-C)负相关。结论肥胖T2DM患者SVCAM-1水平升高,IR程度加重。Objective To investigate serum vascular cell adhesion molecule-1（SVCAM-1） and insulin resistance（IR） in type 2 diabetes mellitus with different body mass index.Methods 130 patients with type 2 diabetes mellitus were divided into three groups： normal body weight group,overweight group and obese group.General condition,lipid profiles,glycometabolism parameters,other serum biochemical parameters and SVCAM-1 were detected and IR was assessed.Results（1）The levels of SVCAM-1 in obese group were higher than in normal body weight group（P〈0.05）,and the levels of Fins,HOMA-IR,IAI in overweight group and obese group were higher than those in normal body weight group（P〈0.05）.（2） It showed that a positive correlations between the serum SVCAM-1 level and SBP,FPG,TG,HOMA-IR and IAI by variable correlations.There was a negative correlation between the serum SVCAM-1 level and HDL-C.Conclusion The obese patients with type 2 diabetes mellitus show an increased serum SVCAM-1 levels and more severity of insulin resistance.
