程序化镇静在预防非计划性拔管中的应用及效果评价

摘　　要：目的通过在儿童重症监护病房（PICU）实施程序化镇静以寻找最安全的镇静技术，以最大限度减少非计划性拔管事件的发生，降低并发症及死亡率。方法对在PICU住院的机械通气患儿，其中326例作为对照组，未使用程序化镇静患儿，统计非计划性拔管发生率；462例为观察组，使用程序化镇静患儿，同样计算非计划性拔管发生率并进行比较。结果观察组使用程序化镇静患儿非计划性拔管率明显低于对照组.χ2=40．76，P〈0．01。结论程序化镇静明显降低了非计划性拔管事件的发生，值得推广应用。Objective To seek the safest sedation technique and maximally reduce unplanned extubation events and reduce the complication and mortality by implementing procedural sedation in pediatrtic intensive care unit （PICU）. Methods Among the children with mechanical ventilation hospitalized in PICU, 326 cases were chosen in control group without asing procedural sedation and 462 cases were in observation group with procedural sedation. The unplanned extubation rates in two groups were calculated and compared. Results The unplanned extubation rate in observation group was significantly lower than that in control group （χ^2=40.76, P 〈 0.01 ）. Conclusion Procedural sedation reduces the incidence of unplanned extubation remarkably, and it is worthy of popularization and application.
