药师干预围手术期抗菌药物使用的效果分析

摘　　要：目的对比评价药师对疝修补术围手术期抗菌药物使用进行全程持续干预前后的效果。方法通过药师实施的一系列措施来改变疝修补术围手术期抗菌药物不合理使用现状,从抗菌药物的适应症、品种选择、用药时机、用法用量等方面进行综合评价干预效果,并与干预前的数据进行比较来验证干预效果。结果经药师干预,医师对疝修补术围术期患者预防用药在适应症、具体品种、用法用量、使用权限、溶媒选择、给药途径、联合用药、给药时机和禁忌症方面较干预前均明显得到规范;人均住院药品总费用占总住院费用的比例由20.47%降至6.19%,人均抗菌药物总费用占总住院费用的比例由11.97%降至0;人均总住院时间由12.3 d降至6.3 d;人均抗菌药物预防应用时间由5.2 d降至0.2d。结论药师对围术期预防应用抗菌药物实施干预是切实可行的,可规范临床抗菌药物的应用。Objective To evaluated the effect of intervention by pharmacists on perioperative prophylactic use of antibiotics in patients with herniorrhaphy. Methods A series of measures were carried out through pharmacists to change the state of peroperatire period irrational using of antibiotics. The intervention effect was evaluated according to the indications, choice of drugs, usage and dosage of antibacterial drugs, and compared to the data before the intervention. Results The intervention by clinical pharma- cist standardized perioperative application of antibiotics in patients with herniorrhaphy. The indication, antibacterial selection, dos- age and usage, drug use permissions, solvent selection, route of administration, drug combination, administration time and contraindication was improved significantly. The proportion of total drug cost in total hospitalization costs decreased from 20.47% to 6.19% ; the percentage of antibiotics in total hospitalization costs decreased from 13.20% to 0;the total hospitalization day decreased from 12.3 days to 6.3days. Duration of antibiotic use decreased from 5.2 days to 0.2 days. Conclusion It was practicable and effective to perform the intervention by pharmacist on the perioperative prophylactic application of antibiotics, which can standardize the clinical use of antibiotics.
