伤椎置钉与跨节段椎弓根螺钉内固定术治疗胸腰椎骨折的比较研究

摘　　要：目的分析研究后路经伤椎置钉与跨节段椎弓根螺钉固定治疗胸腰椎骨折的临床疗效。方法回顾性分析2010年1月至2011年1月56例(观察组)伤椎置钉内固定治疗胸腰椎单一椎体骨折患者的临床资料,随机选择同期跨节段椎弓根螺钉内固定50例作为对照组,术后随访6～24个月。结果所有患者平均随访时间18.5个月。观察组在术后矫正率、术后椎管面积改善值、远期丢失率、内固定失效率方面明显优于对照组,两组比较差异有统计学意义(P<0.05)。结论伤椎置钉内固定治疗胸腰椎单一椎体骨折可以增强内固定系统的牢固性,降低术后矫正角度的丢失及内固定的失败率,是治疗胸腰椎骨折的有效方法。Objective To compare the therapeutic effect and complication of segmental posterior fixation and cross segment pedicle screw fixation for thoracolumbar fracture.Methods A retrospective analysis of clinical data was conducted in 106 patients with thoracolumbar fracture who were admitted in our hospital from January 2010 to January 2011,and they were randomly divided into observation group（56 cases） and control group（50 cases）.The observation group were treated with segmental posterior fixation,the control group were treated with cross segment pedicle screw fixation.Results All cases got the follow-up survey for 18.5 monhts,the rate of rectification,the improve value of vertebral canal area,long-term lost rate and failure rateof internal fixation in the observation group was better than in control group（P0.05）.Conclusion The segmental posterior fixation is an ideal treatment for thoracolumbar fracture and worthy of clinical use.
