摘 要:目的:探讨胎儿Van Praaph-A型永存动脉干的超声心动图表现,对照病理解剖结果,提高产前诊断率。方法:分析2010-08~2011-06在北京安贞医院检出的8例Van Praaph-A型永存动脉干胎儿临床资料、胎儿超声心动图表现及病理解剖结果。结果:应用胎儿超声心动图技术产前共筛查Van Praaph-A型永存动脉干胎儿8例(A1型3例,A2型3例,A3型1例,A4型1例)。其中1例为单纯性永存动脉干,7例合并其它心脏畸形。8例Van Praaph-A型永存动脉干经过病理解剖证实与超声心动图结果一致。结论:应用胎儿超声心动图可以诊断胎儿Van Praaph-A型永存动脉干,而且对于产前筛查合并的其它先心病亦具有重要的应用价值。Objective:To discuss the characteristics of two-dimensional fetal echocardiography,comparison with pathological findings Van Praaph-A type of persistent truncus arteriosus in the fetal as well as the skill of the diagnosis so as to improve the accurate.Methods:Review and analysis the clinic data,two-dimensional and three-dimensional sonogram and autopsy results of the 8 cases with Van Praaph-A type of persistent truncus arteriosus.Results:8 cases of truncus arteriosus were diagnosed by using two-dimensional and spatio-temporal image correlation.According to Van Praagh grouping method,there were 3 of A1 types,3 of A2 types,1 of A3 types and 1 of A4 types.1 case was simple truncus arteriosus,3 cases were complicated with total endocardial cushion defect,1 case with tricuspid atresia,1 case with mitral atresia,1 case with single atrium,2 cases with single ventricle.All of the 8 cases of the truncus arteriosus were confirmed by the autopsy.Conclusion:The fetal echocardiography is a reliable method for the diagnosis in the Van Praaph-A type of truncus arteriosus.The three-dimensional fetal echocardiography is the essential recruitment of the two-dimensional fetal echocardiography,and it is inlportant to the clinical application.
