2型糖尿病患者合并亚临床甲状腺功能减退症患者血脂及hs-CRP水平分析

摘　　要：目的分析2型糖尿病合并亚临床甲减患者血脂及高敏C反应蛋白(hs-CRP)水平,探讨甲状腺功能在糖尿病患者血管病变中的作用。方法采用病例对照研究的方法,对糖尿病合并亚临床甲减组和单纯糖尿病组患者的病变情况、血糖、尿微量白蛋白、高敏C反应蛋白及甲状腺功能等指标进行分析比较。结果合并亚临床甲减组患者总胆固醇、LDL-C、TSH、UAER、hs-CRP水平、收缩压均高于糖尿病组患者,且两组比较差异有统计学意义;32.1%合并亚临床甲减组患者并发心血管病,75.0%患者并发肾病;而单纯糖尿病组患者,11.8%并发心血管病,43.6%并发肾病,两组比较,差异有统计学意义(P﹤0.01)。结论合并亚临床甲减增加了2型糖尿病患者动脉粥样硬化的风险,所以,在糖尿病人群中积极进行甲状腺功能检查,对预防和延缓心血管并发症的发生与发展具有重要的临床意义。OBJECTIVE To investigate the levels of blood lipid and high sensitivity C-reactive protein（hs-CRP）in patients with type 2 diabetes mellitus（DM）complicated with sub-hypothyroidism,so as to explore the clinical significance of thyroid function on angiopathy in type 2 diabetic patients.METHODS The two groups patients,type 2 DM and DM complicated with sub-hypothyroidism,were included by the case-matched study method and their pathologic findings,blood sugar,urinary albumin,hs-CRP,items of thyroid function were detected and analyzed.RESULTS The levels of total cholesterol,LDL-C,TSH,UAER,hs-CRP,systolic pressure in patients with type 2 DM complicated with sub-hypothyroidism were statistically higher than those in the DM patients.There were 32.1% combined with cardiovascular disease,75.0% with nephropathy in patients complicated with sub-hypothyroidism group;While there were 11.8% of concurrent cardiovascular disease,nephropathy 43.6% in patients of DM alone group;There was a significant difference between the two groups（P﹤0.01）.CONCLUSION The complicating sub-hypothyroidism would aggravate atherosclerosis risk in patients with type 2 DM,who should be actively performed the thyroid function test.This prevention and interference measures will be of high clinical value in prediction and delay of concurrent cardiovascular disease.
