肱骨近端骨折的手术治疗方式选择分析

摘　　要：目的分析不同类型肱骨近端骨折的选择手术治疗方式选择的策略。方法选取近5年60例肱骨近端骨折患者为分析对象,分析不同Neer类型骨折手术方式的选择。结果 30例患者行锁定板内固定术,22例患者行肱骨近端解剖钛板固定术,8例患者行人工肱骨头置换术。人工肱骨头置换术后ASES评分稍低(P﹥0.05)。结论加压锁定钢板可用于移位相对较小的Neer二、三部分骨折。对于移位较大的三、四部分骨折应该采用肱骨近端解剖钛板。而人工肱骨头置换适用于严重粉碎骨折不易行内固定术的患者。OBJECTIVE To analyze the surgical strategy for different types of proximal humeral fractures.METHODS In the nearly 5 years,60 cases of proximal humeral fracture patients were selected for analyzing the surgical strategy.RESULTS 30 patients did locking plate fixation,22 patients did proximal humeral anatomy titanium plate fixation,and 8 patients did the head of the humerus artificially replacement.The ASES rating of the patients who did the head of the humerus artificially surgery was lower than the other two groups,but the difference was not statistically significant（P﹥0.05）.CONCLUSION The pressure locking plate can be used to the Neer two or three parts fractures.For the Neer three or four parts patients with the larger shift should be the proximal humeral fractures anatomical titanium plate.And the head of the humerus replacement is applicable to patients with serious fracture and without internal fixation.
