心率变异性Poincare散点图分析与听觉诱发电位指数对麻醉深度监测的相关性研究

摘　　要：目的探讨全身麻醉时心率变异性poincare散点图分析和听觉诱发电位指数(AAI)的变化及相关性。方法选择美国麻醉医师协会(ASA)Ⅰ或Ⅱ级择期手术患者30例,测定手术前1天(T1)、麻醉诱导前(T2)、诱导插管(T3)、术中(T4)、苏醒时(T5)、术后1 d(T6)6个时间点各5 min的心率、平均动脉压(MAP)、散点图短轴(SD1)、散点图长轴(SD2)的变化以及T2～T5时AAI的变化。结果与T1时比较,T6时MAP、SD2明显下降(P<0.05);与T2时比较,T3时心率明显增快,MAP、SD2明显升高(P<0.05);T4、T5时MAP、SD2明显下降(P<0.05);T3～T5时AAI、SD1明显下降(P<0.05)。结论心率变异性非线性分析指标SD1与反映麻醉镇静程度的AAI有很好的相关性。Objective To investigate the changes and correlation between the Poincare scatter plot of heart rate variability（HRV）and auditory evoked potential index（AAI） during general anesthesia.Methods Thirty ASAⅠor II patients with elective surgery were enrolled in this study.HR,MAP,SDl and SD2 of scatter plot were recorded for 5 min at 6 time poins preoperative 1 d（T1）,before induction（T2）,during induction and intubation（T3）,during operation（T4）,analepsia（T5） and postoperative 1 d（T6）.AAI was also recorded at T2-T5.Results MAP and SD2 at T6 were decreased compared with T1（P0.05）.HR,MAP and SD2 were increased at T3 compared with T2（P0.05）.MAP and SD2 at T4,T5 and AAI and SDl during T3-T5 were decreased significantly（P0.05）.Conclusion SD1 as the non-linear anaytic index of HRV reveals good correlation with AAI reflecting the anesthetic sedative degree.
