磁县西部山区一般人群病毒性肝炎血清学调查分析

摘　　要：目的了解磁县西部山区一般人群病毒性肝炎免疫水平及感染状况。方法按照地理区域对磁县西部山区采取多阶段分层抽样方法随机抽取1个乡镇1个行政村,对该村1～59岁居民以户为单位采用系统抽样随机抽取居民户作为调查对象,并兼顾不同年龄组人群,采集204人份血清用ELASA方法检测抗-HAV、HBsAg、抗-HBs、抗-HBc、抗-HCV、抗-HEV。结果被调查人群抗-HAV阳性率为78.43%,随年龄增加而上升,年龄组间差异有统计学意义(χ2=88.53,P<0.01);抗-HBs阳性率为31.86%,抗-HBs阳性率各年龄组间差异有统计学意义,其中1～19岁年龄组阳性率为59.70%,显著高于20～59岁年龄组的阳性率18.25%(χ2=35.61,P<0.01)。HBsAg阳性病例全部为男性,其它检测指标男女间差异无统计学意义。结论本次检测结果显示,磁县西部山区低年龄组甲型肝炎的免疫水平远低于高年龄组,进一步加强儿童甲肝疫苗接种质量,提高免疫水平,对预防甲肝暴发流行意义重大。Objective To understand the immune level and infection status of viral hepatitis among general population in western area of Ci County.Method Adopt multistage stratified sampling method to select one administrative village in a town.In the selected village,taking household as unit,select households as investigated objects by systematic sampling method among residents between 1 and 59 years old.Total of 204 serum samples were collected and detected the anti-HAV,HBsAg,anti-HBs,anti-HBc,anti-HCV and anti-HEV by ELASA.Results The anti-HAV positive rate of investigated group was 78.43%,and the rate increased with age.There was significant difference among different ages（χ2= 89.04,P0.01）.The positive rate of anti-HBs was 31.86%,and it had significant difference among different ages.The positive rate of people between 1 and 19 years old was 59.70%,which was significantly higher than that of people between 20 and 59 years old（18.25%,χ2=35.61,P0.01）.The positive cases of HBsAg were all males,and there was no significant differences between male and female on other detection indexes.Conclusions The test results showed that hepatitis A immunization level among people with low age was much lower than that of people with high age in western mountain area of Ci County.Furtherly enhancing hepatitis A vaccinations quality and improving immunization level to children had great significance in preventing hepatitis A outbreak.
