阿魏酸钠治疗不稳定型心绞痛的疗效研究

摘　　要：目的观察阿魏酸钠(SF)治疗不稳定型心绞痛(UAP)的临床疗效及治疗前后血清中内皮素、一氧化氮(NO)水平的变化。探讨SF治疗UAP的疗效并阐明可能的作用机制。方法 63例UAP患者中男48例,女15例,随机分为两组:常规治疗组(常规组)31例;SF治疗组(SF组)32例。两组患者均给予休息、吸氧、硝酸酯类、β受体阻滞剂、钙拮抗剂、阿司匹林、他汀类调脂药物口服及低分子肝素钙皮下注射,SF组在上述治疗基础上,加用SF 0.3 g溶于0.9%氯化钠溶液250 ml中静脉点滴,1次/d。两组疗程均为14 d。观察两组治疗前后疗效、心电图变化,测定两组用药前后血清内皮素和NO水平。结果两组治疗后与治疗前比较,SF组心绞痛疗效和心电图结果均优于常规组(P<0.05)、血清内皮素水平下降(P<0.05)、NO水平升高(P<0.05);治疗前两组血清内皮素及NO水平比较,差异无统计学意义(P>0.05)。结论 SF在常规治疗UAP的基础上,能够提高临床疗效、改善血管内皮细胞功能。Objective To observe the efficacy of sodium ferulate （SF） in treating unstable angina pectoris （UAP） and explore its underlying mechanism. Methods Totally 63 UAP patients （48 men and 15 women） were randomly divided into con- ventional group （n = 31 ） and SF group （n = 32; receiving sodium ferulate 300 mg/d in addition to conventional treatment）. Both groups were treated for lg days. The clinical effectiveness, electrocardiogram findings, and serum levels of endothelin and nitric oxide were observed and recorded before and after treatment. Results After treatment, the frequency of angina attacks de- creased, the angina duration shortened, and angina attack sublingual nitroglycerin consumption decreased significantly in both groups （P 〈 0. 01 ） ; meanwhile, serum endothelin levels decreased and nitric oxide levels increased significantly （P 〈 0. 05 ）. Compared with the conventional group, the frequency of angina attacks decreased, the angina duration shorten, angina attack sublingual nitroglycerin consumption decreased significantly in the SF group （ P 〈 0. 05 ）. Furthermore, the SF group was signifi- cantly superior than the conventional group in terms of effective rate and electrocardiogram improvement （bothP 〈 0. 01 ）, togeth- er with significantly decreased endothelin level and increased nitric oxide level （ bothP 〈 0. 05 ）. Conclusion SF in combination with conventional treatment is effective for UAP patients, which may be achieved by improving vascular endothelial cell function.
