摘 要:目的研究可用于功能性胃肠疾病治疗的植入式电刺激系统,通过动物实验研究,探讨不同刺激参数对胃肠道收缩活动的作用效果,为治疗用刺激参数的优化提供初步依据。方法系统由便携式体外控制器和植入式体内刺激器组成,体内外通信采用无线模式。刺激脉冲参数及指令由体外控制器设定并发送至体内刺激器,胃肠道收缩活动由体内刺激器集成的压力检测模块采集并发送至体外以供分析。通过电刺激猪盲肠实验,分析不同刺激参数下盲肠压力的变化,以评价刺激参数对盲肠收缩活动的作用效果。结果系统样机工作正常,输出刺激脉冲信号精确,压力信息记录准确。动物实验表明,增加刺激脉冲宽度使盲肠收缩活动的幅度增大,增加刺激脉冲频率使盲肠收缩活动的潜伏期缩短,增加刺激脉冲幅度同时缩短盲肠收缩活动的潜伏期并增大收缩幅度。结论该植入式电刺激系统参数设定范围大,输出脉冲信号精确并具备压力检测功能,便于进行功能性胃肠疾病治疗刺激参数的筛选和验证。动物实验初步验证了不同刺激参数对盲肠收缩活动的作用效果。Objective To develop an implantable electrical stimulation system as an avenue for the treatment of functional gastrointestinal disorders (FGID). To investigate the effects of different stimulation parameters on the gastrointestinal contractile activity in vivo and provide primary basis for the optimization of the stimulation recipes. Methods The system was composed of a portable external controller and an implantable electrical stimulator and they communicated with each other through wireless mode. The stimulation parameters and instructions were sent from the controller to the stimulator while the pressure of the gastrointestinal contraction was recorded by the pressure detection module integrated in the stimulator and was sent to the external controller for the analysis. Action effects of different stimulation parameters on the cecal contraction were verified by the analysis of the pressure in pig's cecum stimulation. Results The system outputted stimulation pulse signals and recorded pressure information with high accuracy. The experiment in vivo indicated that the amplitude of the cecal contraction increased as the stimulation pulse width increased. The latency of the cecal contraction decreased as the simulation pulse frequency increased. The latency decreased and the amplitude of the cecal contraction increased as the stimulation pulse amplitude increased. Conclusions This system provided accurate stimulation pulse signals within a wide range of parameters and was convenient for the selection and verification of the stimulation recipes for the treatment of FGID. The animal experiment verified initially the action effects of different stimulation parameters on the cecal contraction.
