不同频率电针在针药复合麻醉中对肺切除患者心功能的影响

摘　　要：目的观察针药复合麻醉中不同方法对肺切除患者心功能的影响。方法将163例择期行开胸肺切除术患者随机分为A组(常规药物麻醉)、B组(经穴2 Hz)、C组(经穴2/100 Hz)、D组(经穴100 Hz)、E组(经皮穴位电刺激2/100 Hz),分别为31例、34例、32例、34例和32例。采集术前1 d、术中和术后1 d的静脉血心功能的指标(门冬氨酸氨基转氨酶、肌酸激酶、乳酸脱氢酶)进行分析。结果各组门冬氨酸氨基转氨酶、肌酸激酶、乳酸脱氢酶在术前、术中、术后3个不同时间点的差异均有统计学意义(P<0.01)。B组门冬氨酸氨基转氨酶与A组、C组及D组3个时间点的差值比较,差异均有统计学意义(P<0.01,P<0.05)。A组乳酸脱氢酶与B组、D组及E组3个时间点的差值比较,差异均有统计学意义(P<0.05)。B组乳酸脱氢酶与C组、D组3个时间点的差值比较,差异均有统计学意义(P<0.01)。E组乳酸脱氢酶与C组、D组3个时间点的差值比较,差异均有统计学意义(P<0.01)。结论在针刺麻醉4组中,C组、D组、E组门冬氨酸氨基转氨酶优于B组,但不如A组稳定。B组、D组、E组乳酸脱氢酶优于A组。Objective To investigate the effects of different frequencies of electroacupuncture on cardiac function during combined acupuncture and medicine anesthesia in pneumonectomy patients. Methods One hundred and sixty-three elective thoracotomic pneumonectomy patients were randomly allocated to group A （routine drug anesthesia）, group B （acupuncture point 2 Hz）, group C （acupuncture point 2/100 Hz）, group D （acupuncture point 100 Hz） and group E （transcutaneous electrical acupoint stimulation 2/100 Hz）. They were composed of 31, 34, 32, 34 and 32 patients, respectively. Venous blood was taken one day before, during and one day after the operation to measure cardiac function indices （aspartate aminotransferase, creatine kinase and lactic acid dehydrogenase）. Results There were statistically significant differences in aspartate aminotransferase, creatine kinase and lactic acid dehydrogenase between preoperative, intraoperative and postoperative three different time points in each group （P〈0.01）. There were statistically significant differences in the aspartate aminotransferase difference value at three different time points between group B and group A, C or D （P〈0.01,P〈0.05）. There were statistically significant differences in the lactic acid dehydrogenase difference value at three different time points between group A and group B, D or E （P〈0.05）, between group B and group C or D （P〈0.01） and between group E and group C or D （P〈0.01）. Conclusion In the four acupuncture anesthesia group, aspartate aminotransferase is better in groups C, D and E than in group B but not as stable as in group A.
