摘  要:目的通过观察接种后不同时间甲型肝炎(甲肝)疫苗的免疫学效果,为甲肝爆发或流行期间的免疫策略提供参考。方法选择6~10岁甲肝易感儿童作为研究对象,利用随机、盲法原则将研究对象分为4组,分别接种国产甲肝减毒活疫苗(Domestic Hepatitis A Attenuated Live Vaccine,dHepA-L)、国产灭活甲肝疫苗(Domestic Hepatitis A Inactivated Vaccine,dHepA-I)、进口(Import)HepA-I(iHepA-I)、重组乙型肝炎疫苗(酿酒酵母)(Hepatitis B Vaccine Made by Recombinant Deoxyribonucleic Acid Technigues inSaccharomyces CerevisiaeYeast,HepB-SCY)。于接种后1、2、4周检测抗甲肝病毒抗体(Antibody to Hepatitis A Virus,Anti-HAV)。结果接种后第1周,nHepA-L、nHepA-I、iHepA-I的Anti-HAV阳转率分别为7.80%、23.81%、15.79%。几何平均浓度(Geometric Mean Concentration,GMC)分别为5.28、6.87、6.97毫国际单位/毫升(mIU/ml)。接种后第2周,nHepA-L、nHepA-I、iHepA-I的Anti-HAV阳转率分别为85.00%、94.59%和83.33%,GMC分别为29.78、47.42、31.08mIU/ml。接种后第4周,nHepA-L、nHepA-I、iHepA-I的Anti-HAV阳转率分别为97.50%、100.00%、94.59%,GMC分别为32.08、45.81、55.87mIU/ml。结论三种HepA均有良好的免疫学效果。甲肝爆发或流行时,使用nHepA-I可控制疫情。Objective To observe immunogenicity efficacy of hepatitis A vaccines which is vaccinated at different times, and to provide a scientific evidence for formulating immunization strategies. Methods Choose children of 6-10 years old who are susceptible to hepatitis A virus, then randomly divided them into two experimental groups and two control groups. We offered domestic produced hepatitis A attenuated live vaccine (dHepA-L), domestic hepatitis A inactivated vaccine, (dHepA-I),import hepatitis A inactivated vaccine (iHepA-I), hepatitis B vaccine made by recombinant deoxyribonucleic acid technigues in saccharomyces cerevisiae yeast (HepB-SCY)to four groups respectively. The overall positive rates of Antibody to Hepatitis A Virus (Anti-HAV) of the research objects were detected after one, two and four weeks. Blind method was used in the process of experimental research. Result The overall positive rates of Anti-HAV of three groups were 7.80%,23.81% and 15.79% respectively, which were vaccinated with dHepA-L, dHepA-I and iHepA-I after one week, while the Anti-HAV IgG geometric mean concentration (GMC)were 5.28 mlIU/ml,6.87 mlIU/ml and 6.97 mlIU/ml; The rates of dHepA-L, dHepA-I and iHepA-I were 85.00%,94.59% and 83.33% respectively, while the GMC of Anti-HAV IgG of three groups were 29.78 mlIU/ml,47.42 mlIU/ml and 31.08 mlIU/ml after two weeks; The rates of dHepA-L,dHepA-I and iHepA-I were 97.50%,100.00% and 94.59% respectively,while the GMC of Anti-HAV IgG of three groups were 32.08 mllU/ ml,45.81 mllU/ml and 55.87 mllU/ml respectively after four weeks. Conclusion Three kinds of hepatitis A vaccine have good immunogenicity efficacy. The domestic live attenuated hepatitis Avaccines were more efficiently when hepatitis A breake out or prevalent.
