摘　　要：目的了解河南省乙型病毒性肝炎(乙肝)流行现状,评价乙肝疫苗(Hepatitis B Vaccine,HepB)免疫策略的实施效果。方法采用多阶段随机抽样方法,对全省8个县(区)l～59岁人群共3648人进行了乙肝血清流行病学调查,并采用酶联免疫吸附试验对标本进行乙肝病毒(Hepatitis B Virus,HBV)血清学标志物检测,采用复杂抽样调查数据分析方法进行统计学分析。结果河南省1～59岁人群乙肝病毒表面抗原(HBV Surface Antigen,HBsAg)、抗乙肝病毒表面抗原抗体、抗乙肝病毒核心抗原抗体流行率分别为4.63%、51.71%和31.19%。1～4岁人群HBsAg流行率为0.66%,1～14岁人群HBsAg流行率为1.86%。与1992年和2002年乙肝血清流行病学调查结果相比,全人群HBsAg携带率进一步降低,<15岁人群HBsAg携带率下降更明显。结论河南省HepB纳入儿童免疫规划效果显著,人群HBV感染特征已经发生改变,全人群HBsAg携带率持续下降,1～14岁儿童HBsAg携带率下降更为明显。Objective To understand the epidemic situation of hepatitis B in Henan, and evaluate the effect of hepatitis B immunization strategies. Methods A multi-stage randomized sampling was conducted. A total of 3 648 residents aged 1-59 years from 8 counties of Henan province. Hepatitis B virus （HBV）surface antigen （HBsAg）, antibody to HBsAg （Anti-HBs）and antibody to HBV core antigen （Anti-HBc） were detected with enzyme-linked immunosorbent assay method. The survey data was analysized by complex survey analysis technique. Results Among population between 1 and 59 years old. the prevalence rates of HBsAg, Anti-HBs, Anti-HBc were 4.63 %, 51.71% and 31.19 % respectively, but for children between 1 and 4 years old the prevalence rate of HBsAg was 0.66%. The prevalence rate of HBsAg for children between 1-14 years old was 1.86%. The prevalence rate of HBsAg continue to decline compared with the prevalence rates in 1992 and in 2002, especially for children between 1 and 14 years old. Conclusion The survey results revealed that since the hepatitis B vaccine was introduced into the management of expanded program on immunization from 1992, the epidemic characteristic of hepatitis B in Henan province had changed.
