帕瑞昔布对老年全髋关节置换术患者的术后镇痛效果

摘　　要：目的评价帕瑞昔布对老年全髋关节置换术患者术后镇痛的效果及对血液动力学的影响。方法全髋关节置换术患者50例，年龄65～75岁，随机分成两组：帕瑞昔布组和对照组，分别单次静脉注射帕瑞昔布40mg或等容量生理盐水。常规监测平均动脉压、心率。在患者离开手术室前、术后1、6h时评估和记录患者的疼痛评分。结果两组患者术中血液动力学指标差异无统计学意义（P〉O．05）；与对照组相比，帕瑞昔布组术后各时点VAS评分较低，术后需要追加镇痛药的平均时间较长，例数较少（P〈o．05）。结论静脉注射帕瑞昔布40mg可在不影响血液动力学的基础上有效地缓解老年全髋关节置换术患者的术后疼痛。Objective To evaluate the effects of parecoxib on postoperative analgesia and hae- modynamic status for patients after total replacement of hip joint. Methods Fifty patients with total hip joint replacement, aged 65 - 75 years were randomly assigned to parecoxib group and control group, with single dose of intravenous parecoxib 40 mg and same volume of normal saline respective- ly. Heart rate, mean arterial pressure were monitored. VAS was recorded before leaving the operat- ing room, and 1 h and 6 h after the operation. Results There was no difference in haemodynamic in- dexes between the two groups （P^0.05）. Compared with control group, VAS was low in parecoxib group. Average time of postoperative additional analgesics was longer in parecoxib group than that in control group （P（0.05）. Conclusion Intravenous injection of parecoxib 40 mg can relieve postoper- ative pain and not influence the haemodynamics for elderly patients after total replacement of hip joint.
