顺铂、多西他赛序贯腹腔热灌注联合静脉化疗在中晚期卵巢癌围术期的应用

摘　　要：目的观察顺铂、多西他赛序贯腹腔热灌注联合静脉化疗在中晚期卵巢癌围术期中的应用效果。方法42例按照开展腹腔热灌注化疗的时间分为治疗组(2007年7月—2011年1月)25例和对照组(2003年7月—2007年6月)17例,治疗组于行减瘤术前或术后1个月内,彩超引导下行腹腔置管术及顺铂、多西他赛序贯腹腔热灌注联合静脉化疗,并行肿瘤高频热疗,疗程为4～10个周期;对照组行单纯静脉化疗,疗程为6～8个周期。观察两组近期疗效、毒副反应、复发率、无病生存及生存率。结果两组近期疗效、常见毒副反应发生情况和复发率比较差异均无统计学意义(P>0.05);两组生存率和无病生存率比较差异有统计学意义(P<0.05);治疗组无病生存时间和生存时间均较对照组延长。结论中晚期卵巢癌减瘤术前或术后应用顺铂、多西他赛序贯腹腔热灌注联合静脉化疗较单纯静脉化疗疗效较好。Objective To explore the curative efficacy of sequential hyperthermic intracavitary perfusion chemotherapy（HIPEC） with cisplatinum and docetaxel combined with intravenous chemotherapy in patients with advanced ovarian cancer.Methods 42 patients,according to hyperthermic intracavitary perfusion chemotherapy time,were divided into therapy group during July 2007 and Janary 2011（n=25） and control group during July 2003 and June 2007（n=17）.The former were treated before or after one month of cytoreductive surgery with peritoneal catheter instilling operation under ultrasound,sequential HIPEC with cisplatin and docetaxel combined with intravenous chemotherapy for 4-10 periodicities,and the latter received only intravenous chemotherapy for 6-8 periodicities for the purpose to observe the recent effect,adverse reactions,recurrence rate,disease free survival and survival rate.Results The recent effects,adverse reactions and recurrence rates of the two groups were compared,and the differences were not statistically significant（P0.05）,while the survival rate and the disease free survival of the two groups were statistically different（P0.05）;the disease-free survival time and survival time of therapy group were longer than that of control group.Conclusion Intravenous chemotherapy with sequential HIPEC of cisplatin and DOC in treating patients with advanced ovarian cancer leads to superior curative effect.
