耳甲-耳道皮瓣联合颞浅筋膜瓣在完壁式乳突根治术中的应用

摘　　要：目的探讨耳甲-耳道皮瓣联合颞浅筋膜瓣在完壁式乳突根治术中的应用效果。方法本组慢性中耳炎患者28例,采用耳甲-耳道皮瓣联合颞浅筋膜瓣行完壁式乳突根治术,观察临床效果。结果术后耳内创面明显缩小,术后干耳时间为3～4周,外耳道形态正常。22例随访时间6～19个月,耳道及鼓室无炎性渗出或上皮堆积,未发生乳突炎。结论耳甲-耳道皮瓣与颞浅筋膜瓣联合应用可以同时完成乳突填塞和外耳道成形。乳突轮廓化、保护颞浅血管及无菌操作,是手术成功的关键。Objective To explore the effect of auricular concha-meatal flap and superficial temporal fascial flap in canal wall-down tympanomastoidectomy.Methods 28 patients with chronic otitis media underwent canal wall-down tympanomastoidectony,and were repaired by auricular concha-meatal flap and superficial temporal fascial flap.Results All cases obtained dry ear at 3-4 weeks.No recurrence,epithelium accumulation or mastoiditis was observed in 22 patients during the follow-up of 6-19 months.Conclusion The procedure of auricular concha-meatal flap and superficial temporal fascial flap can tamponade mastoid cavity and reconstruct ear canal wall simultaneously.Mastoid outlinization,aseptic technique and protection of superficial temporal vessels are the key points for a successful operation.
