PFN治疗股骨粗隆间骨折31例疗效分析

摘　　要：目的探讨PFN治疗股骨粗隆间骨折的临床疗效。方法2008年2月～2011年4月本科采用股骨近端交锁髓内钉(PFN)内固定治疗股骨粗隆间骨折31例,年龄43～82岁,平均年龄63岁,按Evans分型:Ⅰ型3例,Ⅱ型6例,Ⅲ型8例,Ⅳ型14例。结果随访时间平均11个月,平均愈合时间为4～5个月。根据Harris髋关节功能评分:优17例,良11例,一般2例,差1例,优良率为90.3%。结论股骨近端交锁髓内钉(PFN)内固定治疗股骨粗隆间骨折具有固定牢固、防旋转的优点,创伤小,符合微创技术的治疗要求,是治疗股骨粗隆间骨折的理想内固定选择。Objective To explore the clinical effect of proximal femoral nail （PFN） in the treatment of patients with intertrochanteric femoral fracture. Methods From February 2008 to April 2011, 31 patients aged 43-82 years old （63 years old in average） with intertrochanterie femoral fracture received close reduction and intramedullary fixation using proximal femoral nail （PFN）. According to Evans＇ classification, 3 cases were graded as type Ⅰ, 6 cases as type Ⅱ, 8 cases as type Ⅲ, and 14 cases as type Ⅳ. Results The mean duration of fellow-up was 11 months. The average time of fi-acture healing was 4.5 months. According to Harris hip score system, the results were excellent in 17 cases, good in 11 cases, medium in 2 cases and inferior in l case. The excellent and good rate was 90.3%. Conclusion PFN is an ideal internal fixation for the treatment of intertrochanteric femoral fracture because it can fix fracture rigidly, anti-rotatively, and mini-invasively.
