摘  要:目的探讨不具备血管吻接条件的全头皮撕脱伤者应用全厚皮片回植处理的疗效。方法将撕脱全头皮制成全厚皮片,保留毛囊及厚约1mm的脂肪组织,颅骨裸露处用磨钻磨除颅骨外板,去除多余的头皮片,以使回植全厚皮片与创面基底贴合紧密。结果本组共5例患者,其中4例回植全厚头皮片全部成活,1例大部分成活。随访1—2年,成活头皮片有一定活动度,耐磨,无破溃,且局部有稀疏的毛发生长,效果满意。结论将撕脱头皮制成全厚头皮片回植,具有减少供区继发畸形,成活头皮片耐磨、不易破溃的优点,是处理全头皮撕脱伤的一种较好方法。Objective To explore the effects of full thick skin graft replantation for treating total scalp avulsion injury without possessing conditions of vascular anastomosis. Methods The avulsed scalp skin was made into full thick skin graft with hair follicle and 1 mm thickness adipose tissue reservation. The exposure of skull was stripped to remove the redundant outer plate and scalp so as to make the replanted full thick skin graft and wound bases closed tightly. Results All replanted scalp full-thickness skin grafts survived completely in 4 except one largely survived. After 1 to 2 years following up, all cases were satisfied with a certain activity, wear-resistance, no diabrosis of replanted scalp and sparse hair growth in local area. Conclusion The replantation of full thick skin graft is a good way to repair the total scalp avulsion injury without possessing conditions of vascular anastomosis with such advantages as reducing the secondary deformities in donor site, wear-resistance and no diabrosis.
