肿瘤坏死因子-α单核苷酸多态性与口腔扁平苔藓的相关性研究

摘　　要：目的研究肿瘤坏死因子-α(TNF-α)与口腔扁平苔藓(oral lichen planus,OLP)的相关性。方法应用限制性片段长度多态性-聚合酶链反应(PCR-RFLP)检测151例OLP患者(包括81例糜烂型扁平苔藓和70例非糜烂型扁平苔藓)和143例健康对照者的TNF-α-308位点单核苷酸多态性,分析其基因型和等位基因频率,并进行关联分析。结果TNF-α-308位点的基因型和等位基因频率分布在OLP组与对照组之间无显著性差异(P>0.05),但在糜烂型OLP组与对照组之间比较均有显著性差异(P<0.05);基因型频率的相对风险分析发现,TNF-α-308 TNF2基因型患糜烂型OLP的风险是TNF1的2.268倍(OR=2.268,P<0.05)。结论 TNF-α基因启动子区-308位点的多态性可能不是影响OLP的一个独立因素,但TNF-α-308*A等位基因可能是扁平苔藓局部炎症反应加重的危险因子之一。Objective To investigate the possible relationship between the single nucleotide polymorphisms（SNP） of TNF-α and the susceptibility of oral lichen planus（OLP）.Methods A total of 151 OPL patients（including 81 erosive OLP and 70 non-erosive OLP） who were identified in our department from April to November 2010 and 143 healthy controls who received physical examination in Tiantan Hospital,Beijing from July to October 2010 were enrolled in this study.The polymorphisms of TNF-α-308 were detected in the 151 OLP cases and 143 healthy controls by using the PCR-RFLP technique.The genotype and the allele frequencies were analyzed between the 2 groups.Results There was no significant difference in the genotypepe and allele frequency between the OLP group and the healthy control（P0.05）,but was found between the erosive OLP group and the health control.When compared with healthy control,the allele frequencies of TNF-α-308 ＊A was much higher in the erosive OLP group（P0.05）,and subjects carrying the TNF1 genotype had significantly increased risk of erosive OLP（OR=2.268,P0.05）.Conclusion The polymorphisms of TNF-α-308 may be not a single dependent factor associated with the susceptibility and development of OLP,but the TNF-α-308 ＊A allele might attribute to deteriorate OLP.
