血浆瘦素及5-羟色胺测定在早泄诊断和治疗中的意义

摘　　要：目的:探讨血浆瘦素(Leptin)及5-羟色胺(5-Hydroxy tryptamine,5-HT)水平的测定在早泄(Prema-ture ejaculation,PE)诊断和治疗中的意义。方法:收集71例原发性PE患者服用8周的盐酸舍曲林前后血浆及64例健康男性血浆,采用酶联免疫吸附法检测其Leptin及5-HT浓度。结果:①试验组治疗前血浆Leptin浓度为(32.94±7.72)ng/ml,明显高于对照组(8.84±2.65)ng/ml,两组差异有统计学意义(P<0.05);5-HT浓度为(62.74±18.73)ng/ml,远低于对照组(132.25±41.42)ng/ml,两组差异有统计学意义(P<0.05);而且血浆Lep-tin与5-HT呈负相关(相关系数r=-0.257,P=0.000,差异有统计学意义)。②早泄组治疗后血浆Leptin浓度下降至(7.99±1.86)ng/ml,与对照组比较,差异无统计学意义(P>0.05),而血浆5-HT水平上升至(106.69±38.64)ng/ml,并接近对照组(132.25±41.42)ng/ml,并且血浆5-HT与阴道内射精潜伏期(IELT)呈正相关(r=0.386,P=0.000)。结论:PE患者具有高水平Leptin与低水平5-HT的生物学标记,其测定将为临床医师诊断PE和评估疗效提供一种新方法。Objective：Discuss the meaning of determining Leptin and 5-Hydroxy tryptamine（5-HT） level in the premature ejaculation（PE） diagnosis and treatment. Methods：Collect serums from 64 healthy males and 71 primary PE patients who took sertraline hydrochloride for about 8 weeks, then adopt ELISA to measure their Leptin and 5- HT concentration. Results：（1）The Leptin concentration（32. 94±7.72 ng/ml） before treatment in the trial group is obviously higher than （8.84±2.65）ng/ml in the control group with a statistically significant difference （P〈0. 05）. The 5-HT concentration（62. 74±18.73 ng/ml） in the trial group is far lower than （132. 25±41.42）ng/ml in the control group with a statistically significant difference （P〈0.05）. Leptin and 5-HT have negative correlation （r=- 0. 257, P= 0. 000）. （2）The Leptin concentration dropped to （7.99±1.86）ng/ml after treatment in the PE group without significant difference with the control group（P〉0.05）, and the 5-HT concentration rose to 106.69 ±38.64 ng/ml, close to （132.25±41.42）ng/ml in the control group 5-HT and intravaginal ejaculation latency time（IELT） have positive correlation（r= 0. 386, P= 0. 000）. Conclusions： PE patients have biological mark featuring high Leptin level and low 5-HT level. Determining these levels will provide a new way to diagnose PE and evaluate healing effect for clinicians.
