摘 要:目的评价T2WI-DWI融合技术鉴别诊断前列腺良恶性疾病的价值。方法对经病理证实的13例前列腺癌及43例前列腺良性病变患者行MR检查,由2名医师独立分析图像,先审阅常规MR图像,诊断有无前列腺癌,随后在MR后处理工作站将T2WI与DWI图像融合,根据融合图像提供的信息再次做出诊断。结果 2名医师采用常规MRI及T2WI-DWI融合图像诊断的准确率、敏感度、特异度分别为71.43%(40/56)、76.92%(10/13)、69.77%(30/43)和87.50%(49/56)、84.62%(11/13)、88.37%(38/43),以及60.71%(34/56)、61.54%(8/13)、60.47%(26/43)和80.36%(45/56)、76.92%(10/13)、81.40%(35/43)。融合图像的准确率、特异度明显提高(P均<0.05)。结论 T2WI-DWI融合可提高前列腺癌诊断的准确率和特异度,有助于鉴别诊断前列腺良恶性疾病。Objective To observe the value of fusion of T2WI-DWI in differential diagnosis of benign and malignant prostatic diseases. Methods Thirteen patients with prostate cancer and 43 patients with benign diseases underwent MR exami nation. Two independent radiologists analyzed the images. Firstly conventional MR imaging were assessed and recorded whether prostate cancer was present, then axial T2WI was fused with axial DWI, and the radiologists revaluated for cancer. Results The accuracy rate, sensitivity, specificity of conventional MRI and fused T2WI-DWI for diagnosis pros tate cancer were 71.43% (40/56), 76.92% (10/13), 69.77% (30/43) and 87. 500/00 (49/56), 84. 620/00 (11/13), 88.37% (38/43) for reader 1, while were 60.71% (34/56), 61.54% (8/13), 60.47% (26/43) and 80.36% (45/56), 76.92% (10/13), 81.40% (35/43) for reader 2, respectively, there were significant differences of accuracy rate and specificity be tween the two methods (both P〈0.05).
