中医辨证治疗甲型H1N1流感的临床疗效研究

摘　　要：目的:观察中医辨证治疗甲型H1N1流感的临床疗效。方法:将120例确诊的甲型H1N1流感患者随机分为中医组和西医组,每组各60例。中医组根据中医辨证标准分型后给予桑菊饮加减或银翘散合白虎汤加减,西医组给予磷酸奥司他韦治疗。观察患者的临床症状改善情况及治疗前后T淋巴细胞亚群改变情况。结果:治疗前两组患者病情差异无统计学意义。治疗后临床症状评分中医组为(1.40±0.85)分,西医组为(2.90±1.35)分,差异有统计学意义(P<0.01)。中医组和西医组发热平均时间分别为(3.67±1.13)d与(3.83±1.81)d,差异无统计学意义。每组患者治疗前后T淋巴细胞亚群检测显示CD3、CD4与CD8细胞的绝对数差异有统计学差异(P<0.05),治疗后较治疗前升高。但治疗前与治疗后中医组和西医组比较无统计学差异。结论:中医辨证治疗甲型H1N1流感与西药比较在退热方面疗效相似,对临床症状的改善优于西药,对治疗后外周血淋巴细胞亚群影响与西医组相同。Objective： To observe the therapeutic effect of TCM syndrome differentiation and treatment on novel H1N1 influenza A. Methods： 120 cases of novel H1N1 influenza A were randomly divided into TCM group and western medicine group （n=60）. The patients of western medicine group received oseltamivir phosphate capsules, and the patients of TCM group were given Sangjuyin Decoction plus or minus, or Yinqiao Powder plus Baihu Decoction plus or minus. The improvement of the symptom and physical sign of patients were observed, the subsets of T-lymphocyte was analyzed before and after treatment. Results： There was no difference between two groups in influenza symptoms of the patients before the treatment. The clinical score of influenza symptoms in TCM group was 1.40＋0.85 and in western medicine group was 2.90＋1.35 after treatment, which showed significant difference（P〈0.01）. The mean fever duration was （3.67-＋1.13）d in TCM group and （3.83__.1.81）d in western medicine group, the difference was not significant. Compared with before treatment, the counts of CD3, CD4 and CD8 cells were significantly improved both in TCM group and western medicine group, while there were no difference between the two groups after treatment. Conclusion： TCM syndrome differentiation and treatment has a similar efficacy in shorten duration of fever to western medicine, with better efficacy in improvement of influenza symptom. TCM syndrome differentiation and treatment affected the count of peripheral blood T-lymphocyte subsets as well as western medicine.
