BAK椎间融合器治疗退行性下腰椎失稳症的临床初步报告

摘　　要：目的 :报告一种作者自行设计的新融合术式治疗退行性下腰椎失稳症的临床初步结果。方法 :本组共 15例 ,均为L4 / 5椎间隙退行性失稳症患者。其中男 6例 ,女 9例 ,年龄 42～ 63岁 ,平均 5 2 .1岁。滑脱程度Ⅰ°～Ⅱ° ,L4 / 5椎间隙高度较邻近之正常间隙降低 3 0 %以上。手术方式为侧后方斜向植入单枚多孔螺纹状椎间融合器 (BAK )行腰椎椎体间融合术 (PLIF)。结果 :平均随访 13个月 ,皆达到临床融合。所有患者均无融合器的移位 ,患者主观满意率 90 %。结论 :作者认为 ,经侧后方斜向植入单枚多孔螺纹状椎间融合器行后路腰椎椎体间融合术 ,既能尽可能多的保留脊柱后部结构 ,又能充分完成后路减压 ,并能保证可靠的融合 ,不失为一种比较理想的治疗下腰椎退行性失稳症术式。Objective: To report a newly designed fusion method for treating lumbar degenerative spondylolisthesis.Methods:15 cases of L 4/5 degenerative spondylolisthesis patients were treated with one single threaded hollow fusion cage (BAK) in a posterior lateral approach.Among them 6 were male and 9 female with age ranging from 42 to 63 years old(average 52.1 years).Preoperative radiography suggested a Ⅰ°～Ⅱ°degree of spondylolisthesis and the space of L 4/5 disc decreased more than 30%.Results:13 months follow up shows all 15 cases reach clinical fusion,and no cage migration was found,90% patients got satisied. Conclusions: Posterial interbody fusion with a single threaded hollow fusion cage in a lateral posterior approach can preserve the posterior tissues sufficiently and decompress completely as well as ensure a reliable fusion.The authors thought it was an ideal fusion method in treating lumbar spondylolisthesis.
