自体脂肪颗粒移植隆乳术74例临床分析

摘　　要：目的探讨自体脂肪颗粒注射隆乳的临床效果。方法 2006年1月～2010年12月对74例小乳症患者施行自体脂肪颗粒注射隆乳手术。采用20 ml注射器抽吸自体脂肪,通过清洗纯化处理后移植于乳房腺体内、腺体后及乳房皮下脂肪层。结果本组74例患者术后切口均Ⅰ期愈合。均获随访,时间6月～1年。术后患者乳房体积均有不同程度的增大,多数患者需经过多次注射方能达到满意效果,1、2、3次注射后满意率分别为4.05%、23.88%、85.42%,总满意率为81.08%。术后有2例出现脂肪液化,3例出现乳房小结节。结论自体脂肪颗粒注射移植隆乳手术操作简单,创伤小,效果显著,易被患者接受。在脂肪吸取、纯化及注射过程中严格规范操作,是提高手术效果、减少并发症的有效方法。Objective To investigate the clinical effect of augmentation mammoplasty by autologous fat granule injection. Methods 74 women admitting from January 2006 to January 2010 were performed augmentation mammoplasty by transplantation with autologous fat granule injection. The method was as follows： Autologous fat was sucked by 20 ml syringes, and then transplanted to the inner mammary gland, posterior mammary gland and mammary subcutaneous adipose layer through low temperature layering purification. The efficacy and clinical experience was analyzed. Results All 74 women were gained normal healing of incision in stage Ⅰ, and got a significant improvement in their sizes within 6 months to 2 year follow-up after the grafting. Most patients required repeated injections to achieve satisfactory results. The satisfaction rate after once-, twice- and thrice injection was 4.05%, 23.88% and 85.42% respectively, and the total satisfaction rate was 81.08%. Fat liquefaction was found in 2 cases and improved by paracentesis under B-ultrasonic guiding, and breast nodule was in 3 cases and rechecked regularly without further treatment. Conclusions The transplantation of autologous fat granule injection is a simple, mini-traumatic, effective and acceptive method for augmentation mammoplasty. During the course of suction, purification and injection of fat granule, strict and normative operating is the key point to improve the efficacy and reduce the complications.
