“÷”形切口原位植皮法腋臭根治术

摘　　要：目的介绍一种改良的无明显瘢痕、效果确切的腋臭手术治疗方法——"÷"形切口原位植皮法腋臭根治术。方法 2008年6月～2011年6月采用"÷"形切口原位植皮法治疗双侧腋臭患者57例。于梭形腋毛区中部作3 cm横切口,两端作约0.5 cm小切口,经中部切口于腋毛区皮下进行锐性剥离、弯形薄剪修剪皮下组织及含大汗腺、毛囊真皮深层,另用刮匙经两端切口搔刮剪刀较难到达的腋毛区两端,将腋毛区皮肤修薄成中厚至全厚皮片,原位植皮,加压包扎。结果本组57例,均顺利完成双侧手术,平均手术时间82(65～112)min。完全成活55例,双侧血肿形成致部分皮肤坏死2例,经清除坏死组织和换药处理,于拆线后1月瘢痕愈合。术后随访30例,平均时间8.5(6～12)个月,瘢痕不明显,且无明显异味,效果满意。结论 "÷"形切口原位植皮法治疗腋臭简单可行,效果可靠,无明显瘢痕,值得推广。Objective To introduce a modified radical correction for axillary osmidrosis with skingrafting in situ via ＂ ＋＂ -shape incision approach and to explore its effect. Methods Fifty-seven eases suffered bilateral axillary osmidrosis admitted from June 2008 to June 2011 were treated by a radical treatment. A transverse incision with length of 3 em was made in middle region of fusiform shape armpit hair field and two small incisions with length of 0.5 cm was made in each end of fusiform shape armpit hair field. Subcutaneous tissue, apocrine sweat glands and deep layer of hair follicle and dermis was clipped via the transverse incision of central region and scraped through two small incision of each end of armpit hair field. After skingrafting performed in situ, pressure dressing was carried out. Results All fifty-seven cases were accomplished the radical surgery successfully. The average operative duration was 82 minutes （ranged from 65 to 112 mi- nutes）. Fifty-five cases survived completely. Partial skin necrosis associated with haematoma formation ,;ccurred in both sides of two cases, and healed by clearance necrotic tissue and change dressings one month after disconnecting sutures. Thirty cases were followed up for 6 to 12 months with an average of 8.5 months ; satisfied outcome was achieved in them. Conclusion The modified radical correction for axillary osmidrosis with skingrafting in situ via ＂ ＋ ＂ shape incision approach is simple, feasible and effective without obvious cicatrix, and it is worth popularizing.
